What You’ll Do
The Insight CSC Supported Employment and Education Specialist (SEES) provides assistance to young adults enrolled in the program with obtaining and retaining competitive employment and/or educational setting and builds partnerships with area businesses to connect program enrollees with opportunities.
A facilitator interacts directly with youth and families. The frequency varies with needs, availability of youth, and level of intensity. It is usually ranges from several times a week to weekly and may include availability on evenings or weekends and availability for crisis intervention. The employee also interacts frequently with outpatient therapists, physicians, and other center employees involved in the youth’s care. There is regular interaction with community agencies and providers such as: DCS, court, schools, health care providers, referrals and coordinated services. Caseload expectations will consider training, care complexity, level of frequency, and/or other assigned duties.
Primary duties include:
- Evaluating the family’s need for services
- Developing and monitoring a treatment plan that supports the family’s goals
- Helping the family obtain services needed to support their goals, such as food or housing assistance, medical care, Social Security benefits, transportation, skills training, etc.
- Following up to make sure the family is receiving services and those services are helping
- Advocating for the family and serving as a link between DCS, court, schools, healthcare providers, employers, community agencies and other Oaklawn services
- Documenting all family interaction electronically
- Interacting directly with clients, face to face - frequency varies with need, availability of the client and the level of intensity
- Interacting frequently with outpatient therapists, physicians, other employees involved in the case, community agencies, referrals sources and other services
- Caseload expectations will take into account training, case complexity, level of frequency and/or other assigned duties
- Hours will include availability in the evenings and/or weekends
Minimum Hiring Requirements:
- Two years of college and two years of experience in a similar human services job OR
- BA/BS in social service or a human services-related field preferred
- Two years’ experience in a social service setting preferred
- Must have a valid driver’s license and proof of insurance
