Email/Notes Group Director is responsible for all aspects of the email/notes group. Oversees the design, development, release, and maintenance of email application systems. Being an Email/Notes Group Director requires a bachelor's degree. Typically reports to a top management. The Email/Notes Group Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. The Nurse Practitioner In-Home Assessment are responsible for performing Health Risk Assessments and occasional transitional care for patients discharged from hospitals to prevent re-admissions. As a Nurse Practitioner In-Home Assessment you will:- Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
- Educate and counsel patient and family based on assessment results, including recommendations
- Performs follow up visits in transitional care
- Current RN and NP licensure in the state of practice
- Current CPR certification
- Must be enrolled in both Medicare & Medicaid
