Position Summary:
The Program Manager works closely with departmental directors, managers, and cross-functional project teams to complete Population Health Services Organization project deliverables in accordance with the project plan and to provide documentation, reporting and oversight support. The Program Manager, Population Health is responsible for managing the portfolio of Population Health projects from start to finish, overseeing the execution of these projects within the constraints of budget, schedule and scope while minimizing risk and ensuring adherence to established processes and methodologies.
Responsibilities may include:
- Plans, schedules, coordinates staff members in the planning, designing and maintaining of Population Health project tasks to define technical and business related requirements and assists in the audit and enforcement of such requirements.
- Prepares Population Health project documents including charters, summaries, budgets, or feasibility studies for decision-making bodies such as the executive or Population Health steering committees. Participates in such committees as assigned.
- Manages third-party vendor relations and partners with Information Technology department(s) for information technology applications or processes related to project management.
- Serves as a resource to staff to resolve problems, develop solutions and provide overall guidance. Oversees the performance of assigned staff; assists management with preparation of annual performance reviews; develops and trains assigned staff.
- Provides direction for staff assigned to modify, enhance, upgrade and/or add modules to existing applications.
- Coordinates multiple and/or large projects throughout the life cycle, initiation through closure; manages projects of various scope and size.
- Leads team in creating full-scope project plans and required project documentation that identifies when each task will be performed, timeline for completion, the communication plan and resources required; revises as appropriate to meet changing needs and requirements.
- Works with internal and external stakeholders to ensure project scope, tasks, deliverables, goals, resources, schedules and budgets are explicitly defined, documented, communicated and maintained.
- Provides status reports throughout the project to internal and external stakeholders. Works to resolve issues that arise during the project and apply appropriate measures to correct. Quickly escalates to management any project related issues that will negatively affect project or outcomes.
- Effectively manages project scope by ensuring any changes are documented and approved both internally and externally with project change request forms.
- Identifies and assigns resources required to fulfill project delivery, collaborating with other team members to determine which project resources are available and scheduling project workload and assignments.
- Manages process improvement for Population Health related processes. Suggests areas for improvement in internal business processes and makes recommendations for possible solutions.
- Keeps the project team well informed of changes within the organization that may have a potential effect on project outcomes.
- Ensures project documents are complete, current, and stored appropriately.
- Performs other duties as assigned.
Minimum Qualifications:
- At least 5 years of project management experience managing healthcare or IT projects.
- High School Diploma/GED
Preferred Qualifications:
- Experience working in a healthcare, Information Technology, or other related business environment managing projects across multiple lines of business preferred.
- Bachelor's degree in Business Administration, Healthcare Administration, Computer Information Systems, or other related field preferred.
