As our practice continues to grow, we have a need for additional staff to engage our complex patients with multiple chronic conditions between office visits. The goal is to help them understand and enroll in a program that will provide personalized assistance from a dedicated healthcare professional and coordinate their care between their pharmacy, specialists, testing centers, and hospitals.

    The Chronic Care Management Enrollment Coordinator plays a vital role in facilitating the enrollment process for patients into chronic care management programs. They are responsible for educating patients about the benefits of CCM services, obtaining necessary consent and information, and coordinating with healthcare providers to ensure seamless enrollment. The specialist serves as a liaison between patients, healthcare professionals, and insurance companies to ensure compliance with program requirements and optimize patient outcomes.

    Requirements

    Key Responsibilities:

    • Educate patients about chronic care management services, including benefits, eligibility criteria, and program requirements.
    • Obtain patient consent for enrollment in chronic care management programs and ensure all necessary documentation is completed accurately.
    • Collaborate with healthcare providers to identify eligible patients for chronic care management and obtain relevant medical history and information.
    • Coordinate with insurance companies to verify coverage and obtain authorization for CCM services.
    • Assist patients in scheduling appointments and accessing healthcare resources as needed for chronic condition management.
    • Maintain accurate and up-to-date records of patient enrollment, consent forms and communications with healthcare providers and insurance companies.
    • Provide ongoing support to enrolled patients, including answering questions, addressing concerns, addressing billing issues, and coordinating care as a liaison with other population health teams as needed.
    • Monitor patient participation and compliance with program requirements.
    • Collaborate with interdisciplinary healthcare teams to ensure comprehensive care coordination and continuity for patients enrolled in chronic care management programs.
    • Stay informed about changes in healthcare regulations and guidelines related to chronic care management and ensure compliance with all relevant policies and procedures.

    Required Skills:

    • High school diploma or equivalent.
    • 2 or more years work experience in a similar environment
    • Nationally recognized Medical Assistant (MA) certification or Licensed Practical Nurse (LPN) certification or EMT-B, EMT-I, or EMT-P License from NREMT or GA State Board of EMS
    • Prior experience in care coordination or patient enrollment preferred
    • Strong communication and interpersonal skills, with the ability to effectively educate and engage patients.
    • Excellent organizational skills and attention to detail, with the ability to maintain accurate records and documentation.
    • Proficiency in using electronic health record systems and other healthcare software applications.
    • Knowledge of medical terminology, chronic conditions, and healthcare regulations related to chronic care management.
    • Ability to work independently and as part of a multidisciplinary team in a fast-paced healthcare environment.
    • Compassionate and empathetic demeanor, with a commitment to providing high-quality patient care.
    • Flexibility to adapt to changing priorities and ability to manage multiple tasks simultaneously.
    • Pharmacy technician certification or a minimum of 5 years work experience.
