Advises about types of counseling services available and makes referrals to service providers. Additionally, Family Support Counselor may require a bachelor's degree in human services related field. Typically reports to a supervisor or manager. The Family Support Counselor work is closely managed. Works on projects/matters of limited complexity in a support role. GENERAL DESCRIPTION :
The Family Support Specialist is responsible for providing home-based prevention services focusing on supporting the parent child relationship, attachment, and family enhancement through child development education, play, goal setting, assessments, and coordination of services / activities for overburdened families.
This position reports directly to the HF Supervisor.
SUMMARY OF ESSENTIAL JOB FUNCTIONS :
Conducts home visits virtually or in-person and assesses family needs and provide case management services when necessary, while promoting self-sufficiency.
Participates in outreach efforts to include calls, texts, un-announced drop by’s, mailings etc.
