JOB TITLE : LMHP (LPC or LCSW) or License Eligible Clinical Support Staff
IMMEDIATE SUPERVISOR : Clinical Director
DUTIES AND RESPONSIBILITIES :
Complete face-to-face as assigned by the Clinical Director (23hr, MCR, CS, MHSB, IIH, SA PHP or MH PHP)
Responsible for conducting the initial screening, intake, and diagnostic impression
Complete and submit the SRA and DMAS forms within designated timeframe
Complete the CEPP and obtain all required signatures
Complete prescreens and intake process
Complete initial ISP
Follow up on authorization approvals and conduct peer reviews, as needed
Adhere to all deadlines as specified per program and DMAS as it pertains to submission of documentation (23hr, MCR and CS within 24hrs;
MHSB, IIH, SA PHP and MH PHP weekly)
Make certain the assigned QMHP has the necessary information needed
Confirming the assigned QMHP completes the transition and discharge plan
Provide outpatient therapy services in the office and the community
Case management functions of linking, arranging, and advocating for services and referrals
Maintain up-to-date individual records, and prepare required session notes in a timely and accurate manner
Perform community outreach and advocacy with other public and private agencies / services
Collaborate with Clinical Director during supervisions and monthly staff meetings
Complete reassessments for individuals needing to remain in the program
Completion of the initial treatment plan to identify service needs
Coordinating care with other assigned clinicians, QMHP, Team Lead’s, Supervisor’s, Program Director’s and Clinical Director
Participates in the direct delivery of services, as needed
Attend mandatory monthly staff meetings and all other scheduled meetings regularly
Attend morning huddle meetings each Monday, Wednesday and Friday, via Google meet
Work responsibly with the administrative team and direct supervisor(s)
Professional training 20 hours per year required. Training hours can include but are not limited to the following : Documentation of Orientation, 6 weeks of individual (clinical), 4 weeks of group, administrative orientation, in-service training, and training at other places of employment.
Classes taken should all be documented on the training form. All other training must be approved by Director.

MINIMUM QUALIFICATIONS :
Must be registered as an LPC or LCSW and maintain an active licensure or license eligible and registered with the board, no exceptions.
thorough knowledge of clinical interviewing and assessment skills; working knowledge of human services system, and relevant community resources.
Must be Licensed to provide clinical services by the State of Virginia by the Virginia Department of Health Professions.
A record of license or residency will be housed in the human resource file.
