Job Description:The Business Health Resource Navigator will work in collaboration with care teams to support the efforts to achieve business health initiatives for employer partners. This position works closely with all areas of the Care Team, including providers, patients, and families. Works collaboratively with internal and external partners to advocate for patient needs, to reduce duplication of effort, and to assure service information is current and accessible. Responsibilities include sharing of pertinent patient information with the team, performing outreach and education to patients and families, assisting with care team tasks, and identifying opportunities to enhance care coordination for the employer population. Scheduling appointments and connecting patients with needed services. Other responsibilities include developing, maintaining, using, and disseminating a comprehensive, reliable, and accurate community resource database.
Major Responsibilities:
1. Gathers and maintains a database of available resources in the counties in which Gundersen serves.
2. Coordinates communication and follow-up to patients, providers, community entities, etc., in accordance with plan of care (telephonic, virtual, face to face, or home visits)
3. Identifies and/or develops community resource packets and patient education materials for the purpose of educating patients and caregiver about relevant topics, programs, and resources (appropriate utilization and access to health care services).
4. Coordinates team meetings, agenda, meeting minutes, and follow up.
5. Participates as a team player and attends internal and external meetings as required to provide resource updates to staff.
6. Contacts insurance companies to verify benefits, ensures referrals are covered, and explains alternative medication options
7. Meets with patients and families to assist with completion of necessary paperwork, forms, or resource materials.
8. Contacts pharmacies when needed (assesses adherence, assists with medication reconciliation, bubble packaging)
9. Collects data and compiles reports as needed.
10. Provides expertise in community-based resources for medically complex patients.
11. Maintains equipment and supplies management.
12. Provides age-appropriate care/service as applicable to role.
13. Assures reliable, predictable outcomes and no needless waits.
14. Uses Information Systems to their full capability.
15. Duties as defined by specific department (if applicable)
16. Attends regularly scheduled and assigned shifts
Education and Learning:
REQUIRED
Associate degree in a related field
DESIRED
Bachelor's degree in a related field
Work Experience:
REQUIRED
Two years recent experience in a health care or human service-related field
