JOB SUMMARY
The Nurse Navigator functions on the multidisciplinary team as an advocate, educator, and liaison for oncology and malignant hematology patients. She/he is responsible for ensuring all adult patients with an oncology diagnosis receive quality and comprehensive services. She/he will coordinate patient care throughout the continuum in collaboration with the multidisciplinary team. She/he will serve as a clinical resource with expertise in oncology and malignant hematology care management. She/he will serve as a liaison throughout the facility and the community regarding services provided for this unique patient population. She/he will provide expert nursing care which includes direct clinical practice, research, and education. She/he will serve as a single point of contact for referring physicians to provide resources and assistance with accessing clinical and supportive care services within St. Mary’s Health System. She/he will be flexible to meet the needs of patients.
