Flight Navigator locates the position of an aircraft and directs its course on domestic and international flights, using navigational aids. May require an associate's degree with at least 4 years of experience in the field. Being a Flight Navigator is familiar with a variety of the field's concepts, practices, and procedures. Relies on experience and judgment to plan and accomplish goals. Additionally, Flight Navigator performs a variety of complicated tasks. May direct and lead the work of others. Responsibilities:
The ideal candidate will serve as the designated Patient Care Navigator to an assigned group of patients. The Patient Care Navigator will partner with the patient and care team to reduce readmission risk and support patients' health-related goals through monthly contact. Patient contact may occur by phone, electronic communication, and on-site interactions. Greater than 50% of time will be spent making outbound patient calls. Work may be on-site, hybrid, or fully remote as required to meet client needs.
