Flight Navigator locates the position of an aircraft and directs its course on domestic and international flights, using navigational aids. May require an associate's degree with at least 4 years of experience in the field. Being a Flight Navigator is familiar with a variety of the field's concepts, practices, and procedures. Relies on experience and judgment to plan and accomplish goals. Additionally, Flight Navigator performs a variety of complicated tasks. May direct and lead the work of others. The primary responsibilities of the Breast Care Nurse Navigator are to assist women that have an abnormal examination or a positive finding and/or an increased risk of developing breast cancer. The Nurse Navigator facilitates patient follow-up through patient coaching, information transfer and communication in order to improve continuity of care. The navigator is specifically trained to coordinate the clinical, educational, and supportive needs of patients and their families who are either facing a possible breast cancer diagnosis, newly diagnosed with breast cancer or have an increased risk of developing breast cancer.
RESPONSIBILITIES:
1. Functions as a liaison for management staff, administration, physicians, community organization.
2. Coordinates efforts for patients care plan and promotes quality improvement and educational efforts.
3. Compiles data, tracks outcomes, and makes recommendations for process improvement.
4. Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
5. Integrates patient-centered care in the nursing processes to include the patient(s) and family in care decisions, incorporating evidence based practices to achieve safe and effective patient and process outcomes.
6. Advocates for the unique needs of individuals and the population served: anticipates and removes barriers and act to assure service(s) access and service effectiveness.
7. Communicate with physicians about patients’ potential for referral/follow-up treatment, and schedules follow-up appointments.
QUALIFICATIONS:
Graduate of an accredited school of nursing. Bachelor’s degree in Nursing preferred. Must have 3 or more years of nursing experience. Participates in local (in addition to breast cancer conference attendance), state, regional, or national breast-specific educational programs annually.
