In this role, you'll schedule and lead Planning Meetings, collaborate with Medicaid Service Coordinators, and champion individualized Self-Directed Plans. Actively seek community connections, develop tailored budgets, and maintain open communication with supervisors, support circles, and service providers. Document progress diligently, engage in professional development, and contribute to our mission of fostering an inclusive and supportive community.
Work Location: Canandaigua
Schedule: 8am to 4pm with flexibility to meet program needs
