    This job is used to reduce the use of emergency departments for non-emergent reasons. This job will achieve streamlined patient care transitions and redirection to appropriate levels of care utilizing hospital and community resources to effectively educate and empower patients and their families.

    Education

    Required - Associate's degree with required related experience

    Preferred - Bachelor's degree

    Work Experience

    Required - 2 years of experience in social services or clinical setting with knowledge of community resources

    Preferred - Experience in case management, community based organizations, or health care organizations working directly with clients

    Knowledge Skills and Abilities (KSAs)

    • Must have computer skills and dexterity required for data entry and retrieval of information.

    • Effective verbal and written communication skills and the ability to present information clearly and professionally.

    • Must be proficient with Windows-style applications, various software packages specific to role and keyboard.

    • Strong interpersonal skills.

    • Strong oral and written communication skills.

    • Proven leadership skills, ability to think critically, problem-solve, work independently, self-initiate, multi-task, and work effectively in a team environment.

    • Highly organized and detail-oriented.

    Job Duties

    • Conducts Initial Evaluation and Engage Patient/Family.

    • Identifies Patient Needs and Connect Patients to Appropriate Resources.

    • Utilizes Behavior Change Strategies to Educate Patients and Families.

    • Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

    • Other related duties as required.

    Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. The risk level of exposure may increase depending on the essential job duties of the role.
