We are seeking a highly motivated LVN/LPN with at least 1 year of experience in home health to join our team on a 13-week contract basis. The ideal candidate will have a valid CA license, Oasis and HCHB experience, and a flexible schedule. The LVN/LPN will be responsible for providing high-quality care to patients in the Orange County coverage area. The LVN/LPN must have reliable transportation and a valid driver's license.

Description:

A field position is open for a LVN/LPN for Home Health coverage in the Orange County area of San Diego, CA. This is a full-time contracted position with guaranteed weekly hours and a salary of $40/hr. The shift is from 9:00 AM to 6:00 PM with flexibility, and an on-call rotation every three weeks. The contract duration is for 13 weeks.-renewable-

Responsibilities:

• Conduct initial and ongoing assessments of patients
• Develop and implement care plans based on assessments
• Administer medications and treatments as ordered by the physician
• Document patient progress and report changes in condition to the physician
• Provide education to patients and families on disease management, medication management, and other healthcare topics
• Communicate with physicians, healthcare team members, and family members
• Maintain accurate and up-to-date patient records in compliance with state and federal regulations
• Adhere to all HIPAA regulations and maintain patient confidentiality

Qualifications:

• Valid LVN/LPN license in the state of California
• At least 1 year of experience in home health
• Oasis and HCHB experience preferred
• Ability to work independently and with a team
• Strong communication and interpersonal skills
• Ability to handle multiple tasks and prioritize effectively
• Flexible schedule
• Reliable transportation and a valid driver's license
• COVID vaccination preferred - Exemptions permitted with weekly testing

Other Details:

• Must have a CA license in hand
• Minimum of 1-year Home Health experience required
• Oasis experience preferred
• HCHB experience preferred but will train.
• Mileage at IRS standard rate between patients.
