Foundation Director directs the execution of the vision, strategic planning, financial management, fundraising, and grantmaking operations that support the overall objectives of a foundation. Defines the goals and tactics for fundraising campaigns and program initiatives. Being a Foundation Director represents the foundation to the public and media to communicate its mission, values, and accomplishments. Maintains positive relationships with foundation stakeholders, including the board of directors, grantees, community leaders, donors, and prospective donors. Additionally, Foundation Director oversees grantmaking operations, including developing grant guidelines, reviewing grant applications, and managing the grant evaluation process. Manages foundation programs and initiatives that align with the foundation's mission and values. Implements systems for monitoring and evaluating the effectiveness of the foundation's grantmaking and programs. Ensures all foundation activities comply with applicable laws, reporting requirements, and regulations. Typically requires a bachelor's degree. Typically reports to senior management. The Foundation Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Foundation Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. Job responsibilities:
A. Participate in the decision to recertify and discharge patients to/from Hospice.Visit patients in home to assess recertification appropriateness for hospice and make
B. treatment recommendations. Will write treatment orders as appropriate.
C. Work with the interdisciplinary care team to assure that the medical needs of the client are met.
D. Documentation of recertification of a patients eligibility in the Hospice program.
E. Work in cooperation with the attending physician to develop and update the plan of care.
F. Serve as a resource to hospice staff, patients, families and attending physicians regarding pain and symptom control measure.
G. Attend IDT meetings as required.
H. Be available to interdisciplinary team members who have questions regarding the prognosis or care of Hospice patients.
I. Provide educational in-services to physicians, facilities and community at large.
J. Make routine home nursing visits as needed.
K. Periodic chart reviews.
L. Performs patient/family teaching; utilizes teaching tools and handouts; documents all teaching and patient response
Additional Skills:
? Communicate effectively orally and in writing.
? Assess patients prognosis with regard to recertification of terminal illness.
? Provide leadership and guidance to the Hospice team.
? Job requires operation of an automobile and the ability to enter and exit automobile.
? To represent Hospice philosophy of care to the medical community.
? Participate in educational seminars and programs as appropriate to the overall educational needs of the Karen Ann Quinlan Hospice.
QUALIFICATIONS:
A. Education: Board-licensed nurse practitioner, licensed to practice in
NJ and PA.
Training and Experience: Experienced in the practice of family practice, oncology or hospital practice.
Job Knowledge: Knowledge of the hospice philosophy of care and the delivery of medical care at home.
