    POSITION SUMMARY

    The Case Manager is dedicated to prevention education targeting youth at risk of runaway or homelessness, suicide, child abuse, child sexual exploitation, and teen pregnancy and parenting in McHenry County. Prevention education happens in school- and community-based settings on three support tiers: one-time classroom/community presentations, 12-week small group counseling, and ongoing individual case management. Additionally, the Case Manager is responsible for networking with other service providers, securing and maintaining contracts for services, and for submitting accurate documentation for those services.

    As a part of the Prevention department of the Behavioral Health pillar, the Case Manager will adhere to the Mission, Vision, and Values of the Pioneer Center and philosophy of the Behavioral Health pillar. This philosophy emphasizes Trauma-Informed Care, Evidence-Based Practices, Strength-Focused Treatment, and Client-Centered Services. To achieve these standards, the Case Manager and all members of the Behavioral Health pillar must attend ongoing internal and external trainings and contribute to a culture of collaborative learning.

    *10-month position, reasoning: As the Prevention Department primarily serves students and partners with schools and districts, the Case Manager role follows the school year calendar.


    ESSENTIAL FUNCTIONS

    Essential Functions Statement(s)

    • Provides school- and community-based prevention education to children, adolescents, and their families
    • Consistently spends 60% of time providing direct service to clients (typically 24 of 40 hours per week)
    • Complete all documentation accurately and concurrently, which is defined by within 24 hours of service delivery
    • Learns and implements Trauma-Informed Care and Evidence-Based Practices and Curriculum
    • Establish and maintain school and community contracts for services
    • Present on topics such as Suicide Prevention, Runaway and Homeless Youth, Child Sexual Abuse Prevention/Erin’s Law, Teen Dating Violence, and other topics as requested by school districts or community partners
    • Facilitate 12-week groups for youth at risk of runaway, suicide, drop-out, or justice involvement; and expectant and parenting teens
    • Maintain a caseload and provide case management services in compliance with Medicaid Rule 132/140
    • Uses strength-focused treatment planning to collaboratively create goals and objectives with clients
    • Completes IM CANS (Illinois Medicaid Comprehensive Assessment of Needs and Strengths) with all clients minimally every 180 days
    • Travels to clients’ homes, schools, and other community locations to meet with clients
    • Makes referrals and advocates for clients in need of assessments, treatment, appointments, and other services
    • Assists clients in accessing public benefits (TANF, SNAP, SSDI, etc.)
    • Teaches and models healthy social skills and independent living skills
    • Develops linkages in the community by networking with other service providers
    • Facilitates or participates in case consultation or staffings with other service providers
    • Provides service in a professional manner and communicates with staff, clients, families, and other agencies involved with the client.
    • Completes required agency training.
    • Provides service according to policy and procedure (HIPAA, Confidentiality, Ethics, etc.).
    • Regular, reliable, predictable attendance.
    • Performs all other duties as assigned by Supervisor.

    SKILLS & ABILITIES

    Education:

    Bachelor’s Degree Required

    Field of Study: Social Services, Social Work, Counseling, Psychology or related field

    Experience:

    Minimum two years of experience in mental health or human service setting

    Computer Skills:

    Electronic Health Record, Microsoft Office, Word, Excel, Powerpoint, Outlook, Teams, Telehealth, Virtual Meetings and Presentations

    Must have and maintain a valid driver's license.

    Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits:
    $50,000 bodily injury, each person
    $100,000 bodily injury, each accident
    $50,000 property damage

    Ability to use Phone, Fax, Copier, and file required.

    WORK ENVIRONMENT

    Work may be performed in a variety of settings with varying levels of cleanliness, noise, etc. Frequent traveling to schools within the county. Frequent interaction with any stakeholder within the agency (i.e. supervisors, co-workers, clients, general public, outside agencies, etc.).
