    POSITION SUMMARY

    The Director of Lifestyle Modification and Prevention Medicine uses the knowledge, principles, and practices of wellness and well-being to develop various wellness and prevention programs for TWCCH clients, community partners, and for the internal TWCCH/TWCGME wellness program. The Director of Lifestyle Modification and Prevention Medicine is a strategic partner who collaborates with TWCCH and TWCGME leadership to seek and promote a culture of health and well-being, provide innovative and science-based solutions, and address the wellness needs of our TWCCH population.
     

    Having specialized expertise in the area of wellness with demonstrated success working in the field, the Director of Lifestyle Modification and Prevention Medicine will assess the TWCCH population lifestyles, address health concerns and work with the population to make behavioral, nutritional and other changes in order to promote better health and wellness. 

    The Director of Lifestyle Modification and Prevention Medicine will oversee the design, development, implementation and long term sustainability of the nutritional service line and other preventable services for patients, including but not limited to health education, tobacco cessation, and other group therapies. The Director of Lifestyle Modification and Prevention Medicine is responsible for leading and managing wellness and preventative programs and is an advocate for integration of wellness data and services into the continuum of care model through collaboration with a multitude of departments including Human Resources, Clinical Operations, and The Wright Center for Graduate Medical Education.

    ESSENTIAL JOB DUTIES & FUNCTIONS

    This position will perform the following duties while living and demonstrating the Core Values.   

    • Responsible for the development and oversight of TWC’s Lifestyle and Nutrition service line which includes developing strategies and programs for continued sustainability. 
    • Work with clinical operations, finance and billing to ensure the service line is meeting organizational goals and objectives.
    • Ensure staff are meeting performance metrics to maintain sustainability and patient satisfaction.
    • Leads, directs, implements and evaluates The Wright Center for Community Health (TWCCH) sponsored public health and education programs, with a focus on community outreach and increasing awareness of the organization’s mission in its five-county service area
    • Evaluates the program’s effectiveness and reviews both participant evaluations and comments
    • Evaluates public health legislation, regulations, executive directives, and departmental policies to determine their relevance to the organization's plans, programs, and activities
    • Leads departmental meetings in a consultative and subject matter expert role for short and long-term design or wellness program offerings
    • Directs the review and revision of the electronic health record software goals and objectives for the preventative care measures, such as obesity, immunization rates
    • Ensure wellness programming is consistent with established goals and is outcome focused
    • Drives the alignment of wellness initiatives and programs with client and community needs, goals, and objectives
    • Analyzes data to measure success of the wellness programs and initiatives on a set periodic basis
    • Oversees the writing and reviewing of policies, procedures, and methods relative to the programs
    • Maintains understanding of organizational goals and referral development activities necessary to meet those goals
    • Implements and evaluates procedures, processes and protocols which support the organization’s goals and objectives of quality of care, resident education and sharing of best practices according to the tenants of the Patient Centered Medical Home
    • Implements actions for programs improvement
    • Consults and collaborates with departmental leadership and collegiate partners to align goals pertaining to the wellness program and initiatives
    • Collaborates with The Wright Center for Graduate Medical Education to support the Lifestyle Medicine Curriculum implementation and development sustainability plan
    • Interpret and relay educational biometric screening results and significance of results to participant and client
    • Uses data analytics to close the gap and drive prevention and wellness related to CV risk, diabetes, obesity reduction, cancer screening, vaccination, nutrition, tobacco cessation.
    • Acts as an ambassador to the TWCCH in the community; establish and maintain working relationships with regional and community partners
    • With the TWCCH/TWGME, maintains electronic health record visit templates related to preventive goals and objectives
    • Continuously review current patient preventive screenings and revise as needed.  Ensure all regulations and guidelines are met, as required
    • Develop and implement patient education materials and training as pertaining to all preventative measures and lifestyle modifications
    • Participate, as needed, at all community events and educational training held for staff as well as patients
    • Educate residents/fellow on lifestyle medicine nutrition, tobacco cessation and other nutrition service relate programs as part of whole person care
    • Develops sustainability plan for a comprehensive and multi-source revenue stream for widespread resources throughout the organization.
    • Develop a workflow and education plan to combine forces with nursing staff for comprehensive whole person care within chronic care management for patients. 
    • Work closely with strategic partners, like AHEC, for an expanded network for organizational growth and employee development.
    • Lead the partnership with agencies such as AHEC, ALA, AHA, ADA for the delivery of tobacco cessation, diabetes education and other programs
    • Responsible for the oversight and leading a community-based certified diabetes education program for community benefit
    • Develop and implement comprehensive cardiovascular risk reduction services for adult population to include blood pressure control/reduction strategies to address hypertension and overall cardiovascular risk
    • Responsible for the oversight of HIV clinical nutrition services
    • Establish and implement company policies related to wellness (e.g., smoking, cafeteria, vending machines)
    • Collaborate with other Program Managers and Wellness Coordinators to refine wellness program objectives that impact client and participant needs and improve engagement
    • Monitor Public Health Preventative Screenings for eligible TWCCH patients. Work with Wellness Coordinators and Case Managers to increase awareness of, facilitate patient access, and follow-up to completion
    • Direct the compilation and updating of electronic health records related to Lifestyle Management Curriculum (LMC) 
    • Coordinate tracking of the residents and faculty requirements in terms of LMC
    • Coordinate in collaboration with Penn State Project ECHO in arena of health living promotion
    • Lead the opportunity for health - promoting disease - preventing message of plant-based nutrition to the patients and the community.
    • Defines the overall philosophy and strategic objectives to align with higher organizational mission
    • Manages department staff, leads weekly staff meetings, conducts performance evaluations on all staff, but is not limited to, annual evaluations, performance improvement plans and disciplinary actions
    • Responsible for identifying staff needs, conducting interview, and recommending new employees in conjunction with medical director and HR
    • Monitor time off
    • Establishes priorities, schedules, task completions, and meets deadlines as evidenced by timeliness of reporting data for operations, grant activation and balanced scorecard review
    • Develops procedures for evaluating the effectiveness of the program’s treatment process to assure that program goals and objectives, learners’ needs, patients’ needs and compliance with all applicable quality standards as well as federal state and local licensure requirements, codes and regulations are being met
    • Provides oversight and guidance to staff to maximize efficiency and productivity
    • Develops processes that improve workflow, patient flow, continuity of care, and resident education
    • Facilitates participation in quality initiatives which includes the development and upload of data reports after thorough review for accuracy for deficiencies.
    • o Evaluates successes and deficiencies to create a comprehensive plan of action utilizing PDSA (Plan, Do, Study, Act) methodology
    • Responsible for completion of all patient referrals and follow ups for nutritional and lifestyle modification needs
    • Responsible for LSM data analysis and management
    • Responsible for collaborating with other members of the team for integrated care
    • Responsible for LSM Management of 20 patients with chronic conditions such as Diabetes Mellitus, weight loss, elevated BMI, hypercholesterolemia
    • Responsible for reviewing and documenting the education plan with patients to include use TWC-specific handouts that address basic disease information
    • Documents patient encounters and contacts made on behalf of patients in EHR; completes and submits monthly reports; maintains comprehensive electronic patient files, which include patient notes, release of information, assessments and other medical documents acquired on behalf of the patient.  
    • Responsible for assisting patients with self-management goal setting to improve healthy behaviors and manage chronic illnesses or conditions.
    • Responsible for billing the current and new codes related to weight loss, BMI, diabetes management, hypercholesterolemia and education of residents on the new code.
