Head Nurse - Quality Improvement - Home Care develops and implements various activities to improve overall quality of organization. Participates in and serves as a resource to quality improvement committees. Being a Head Nurse - Quality Improvement - Home Care tracks and analyzes trends and outcome reports and advises management on areas to improve upon. May be responsible to ensure all institute or government regulations are adhered to. Additionally, Head Nurse - Quality Improvement - Home Care requires a bachelor's degree. Typically reports to top management. The Head Nurse - Quality Improvement - Home Care supervises a group of primarily para-professional level staffs. May also be a level above a supervisor within high volume administrative/ production environments. Makes day-to-day decisions within or for a group/small department. Has some authority for personnel actions. Thorough knowledge of department processes.     Licensed Practical Nurse (LPN):

    A licensed practical nurse (LPN) is a nurse that performs basic medical tasks, including checking vital signs and feeding patients. They’re also responsible for maintaining a clear line of communication between a patient, their family and their caregivers. LPNs work under the direction of registered nurses (RNs) and doctors.

    Qualifications:

    · Possesses a current license to practice as a practical nurse in Ohio.

    Responsibilities:

    · Participates in the planning and coordination of total client care in conjunction with the registered nurse and the physician’s Plan of Care.

    · Follow the nursing care plan for each assigned client.

    · Observes the client, evaluates the care of the client and applies his or her knowledge of nursing skills to the clients to whom assigned.

    · Accurately reports and records the client’s condition and care, including signs and symptoms which might be indicative of change.

    · Prepares clinical/ progress notes of all care given and submits them weekly to the Agency.

    · Provides and maintains a safe environment for the client.

    · Provides teaching to clients and families as necessary.

    · Participates in care conferences and staff meetings as necessary.

    · Performs within limitations of preparation and experience.

    · Keeps supervising registered nurses and/ or Director of Nursing advised of any changes in patient condition, progress, problems, complaints or any unusual occurrences.

    · Performs treatments as assigned per physician’s Plan of Care. An LPN may perform only those treatments for which they are qualified by training and experience to perform.

    Organizational Relationship:

    · Reports directly to the staff nurse and indirectly to the Director of Nursing.

    Functional Abilities:

    · Must be able to read 12 point type or larger.

    · Must be able to communicate verbally and in writing in a manner understood by most persons.

    · Must be able to stoop, bend and be able to lift and transfer clients.

    · Must be able to travel to client’s residences.

    Special requirements:

    · Must have a reliable car, car insurance and a valid driver’s license.

    · Must be able to successfully pass a background check.
