Licensed Practical Nurse (LPN):
A licensed practical nurse (LPN) is a nurse that performs basic medical tasks, including checking vital signs and feeding patients. They’re also responsible for maintaining a clear line of communication between a patient, their family and their caregivers. LPNs work under the direction of registered nurses (RNs) and doctors.
Qualifications:
· Possesses a current license to practice as a practical nurse in Ohio.
Responsibilities:
· Participates in the planning and coordination of total client care in conjunction with the registered nurse and the physician’s Plan of Care.
· Follow the nursing care plan for each assigned client.
· Observes the client, evaluates the care of the client and applies his or her knowledge of nursing skills to the clients to whom assigned.
· Accurately reports and records the client’s condition and care, including signs and symptoms which might be indicative of change.
· Prepares clinical/ progress notes of all care given and submits them weekly to the Agency.
· Provides and maintains a safe environment for the client.
· Provides teaching to clients and families as necessary.
· Participates in care conferences and staff meetings as necessary.
· Performs within limitations of preparation and experience.
· Keeps supervising registered nurses and/ or Director of Nursing advised of any changes in patient condition, progress, problems, complaints or any unusual occurrences.
· Performs treatments as assigned per physician’s Plan of Care. An LPN may perform only those treatments for which they are qualified by training and experience to perform.
Organizational Relationship:
· Reports directly to the staff nurse and indirectly to the Director of Nursing.
Functional Abilities:
· Must be able to read 12 point type or larger.
· Must be able to communicate verbally and in writing in a manner understood by most persons.
· Must be able to stoop, bend and be able to lift and transfer clients.
· Must be able to travel to client’s residences.
Special requirements:
· Must have a reliable car, car insurance and a valid driver’s license.
· Must be able to successfully pass a background check.
