Head of Home Care leads and directs the daily operations of a home health care agency. Establishes and implements quality standards for patient care. Being a Head of Home Care measures and monitors patient satisfaction and outcomes. Ensures that all operations and reporting requirements comply with regulations. Additionally, Head of Home Care requires a bachelor's degree. Typically reports to senior management. The Head of Home Care manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Head of Home Care typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     • Covid-19 vaccination required by Head Start Performance Standards  

     

    Description

    The role of an Early Head Start Home Visitor is to provide in-home instruction as well as a group socialization experience to prenatal parents and children (infants/toddlers) which will enhance the parents’ ability to fill the role of primary nurturer of their child. Maintain on-going contact with families and work with all other components (health, nutrition, family services, mental health, special services, parent education and resource/referral) to integrate services into the family setting. Must adhere to requirements per Federal Head Start Performance Standards and any local, state and/or agency policies and procedures.

     

    Primary Responsibilities: 

    • Provide weekly 90-minute home visits with each family within the child’s home or agreed upon location. Plan parent child activities with parent input, that allow parents the opportunity to observe their children and to engage in parent child interactions. 
    • Create and maintain respectful partnerships with families.
    • Deliver health, educational, nutritional and child growth and development information to parents via weekly home visits.
    • Coordinate with parents to ensure that prenatal appointments, well-child exams, immunizations, health screenings, and nutrition questionnaires are completed and necessary follow-up occurs.
    • Complete family partnership agreements, developmental screenings and all other required assessments with the families in accordance to Head Start Performance Standards.  
    • Support families in locating community resources that best meets their needs and assist in the referral process.
    • Provide support for parent and child activities that encourages developmentally appropriate activities for the changing needs of infants and toddlers.
    • Work with families to develop and support education and family goals based on the needs of the family. This includes reviewing goals and updating individual 

     

     

    plans designed to promote current learning and attend to nutritional, health, and social needs.

    • Maintain accurate written/electronic records including: enrollment paperwork, health information, assessments, disability documentation, child observations, and other required forms/documentation as required in the Head Start Performance Standards.
    • Recruit new families by maintaining contacts and networking with community service providers in the area and assist in the enrollment, recruitment, selection, eligibility and attendance (ERSEA) process for Early Head Start Families.  
    • Plan and implement socialization opportunities for children and families.
    • Implement a program in a manner consistent with EASTCONN mission, vision, code of ethical conduct, policies and procedures. 
    • Demonstrate competency to plan and implement home-based learning experiences that ensure effective implementation of the home visiting curriculum and promote children's progress across the standards described in the Head Start Early Learning Outcomes Framework: Ages Birth to Five, including for children with disabilities and dual language learners, as appropriate, and to build respectful, culturally responsive, and trusting relationships with families. 

     

    The following functions are essential in the performance of this position. Applicant must have ability to:

    • Maintain accurate records and timely completion of required paperwork. 
    • Maintain confidentiality. 
    • Have access to reliable insured transportation. 
    • Obtain a physical exam (statement of good health) with tuberculosis test upon hire and updated every two years. 
    • Physically able to interact with children birth to five, which includes lifting, bending and carrying.
