Job Summary:
We are seeking a highly organized and detail-oriented Care Coordinator to join our team. The Care Coordinator will be responsible for providing administrative support and coordinating patient care. This role requires strong communication skills and the ability to work in a fast-paced medical office environment.

```Responsibilities:```
- Schedule and coordinate patients and caregivers
- Obtain and verify patient insurance information
- Assist with medical record management and documentation
- Coordinate referrals to specialists and other healthcare providers
- Communicate with patients, families, and healthcare professionals to ensure continuity of care
- Provide administrative support to healthcare providers as needed
- Maintain confidentiality of patient information

```Skills:```
- Proficient in medical office procedures and practices
- Knowledge of medical terminology and coding
- Strong computer skills, including experience with electronic health records systems (HHAeXchange)
- Excellent organizational and time management skills
- Ability to multitask and prioritize tasks effectively
- Strong communication skills, both written and verbal
- Attention to detail and accuracy
