Position Summary:
Under the direction of the physician, the Diabetes Educator is responsible for instructing individuals (in-patient, outpatient and community members) concerning their condition, general health management and nutrition, and distribution of diabetic educational literature. Provides information that is relevant to coping with illness, promoting recovery to healthful living and effectively involving the consumer in his/her health care decisions.
Responsibilities may include:
- Assesses individualized educational, behavioral, and clinical needs, facilitates goal planning, provides patient centered education and evaluates desired outcomes. Demonstrates by documentation of individual assess, goals, intervention, evaluation of outcomes.
- Collects and interprets complex data, recognizes subtle changes in the patient/family condition. Demonstrates through documentation.
- Provides individualized care in all aspects of diabetes self-management to patients and their families, as demonstrates through patient feedback, and documentation.
- Participates in the development and implementation of new programs which include: outpatient services, inpatient consultation, and external community and professional programs. Demonstrates by leading or participating in develop. or revision of a service.
- Participates in self-directed work teams to evaluate existing programs, identify need for new programs, and assist in development or revision as appropriate. Demonstrates by actively carrying out required work assignments.
- Develop new patient educational materials. Demonstrates by revising or developing at least one educational material.
- Participates in community and professional outreach that increases overall awareness of diabetes. Demonstrates through participating in the planning of one department sponsored community or professional program, as well as present at one program annually.
- Other duties as assigned.
Minimum Qualifications:
- Three to five (3-5) years of clinical experience. Experience in Diabetes self-management education.
- Clear and current CA Registered Nurse (RN) or Registered Dietitian (RD) license.
- Bachelors degree.
- CDCES/CDE within 1 year or 500 hours of direct diabetes education within the last 2 years.
- Membership in local professional organizations.
- Ability to work independently, set priorities and manage work within timeframe, read and analyze technical material, communicate effectively verbally and in writing, positive mental attitude.
- Strong customer service orientation and the ability to handle highly confidential information essential.
- Contact hours, continuing education, specialty training in diabetes self-management education
Preferred Qualifications:
- 5 years of clinical experience.
- CDCES/CDE
