Health Educator is responsible for developing and designing health education programs meant to increase community awareness and knowledge. Conducts needs assessments and keeps up-to-date on changes in health care technology to keep program material current. Being a Health Educator develops, coordinates, and disseminates educational resources and materials. May provide health education outreach and consultation. POSITION PURPOSE:
The Peer Specialist works as a member of the CCBHC team provides linkage to mental health treatment and supportive services for adults with severe mental illnesses. Maintain an active caseload of clients who need case management and supportive counseling on an ongoing basis.
RESPONSIBILITIES:
• Provide support services to program participants and serves as a member of the Jail Diversion Program staff.
• Assist program participants with the jail release process.
• Accompany program participants to appointments in the community and court using public transportation.
• Provides information to program participants, staff and other stakeholders as necessary.
• Document interactions with program participants utilizing the Jail Diversion Program Service Point computer system.
• Serve as consultant and faculty for Crisis Intervention Team training to law enforcement personnel.
• Handle multiple assignments simultaneously, including providing assistance to Jail Diversion staff as necessary.
• Performs related work as required.
POSITION REQUIREMENTS / QUALIFICATIONS:
Education/Experience: High School diploma or GED required. Certified Recovery Peer Specialist preferred but will consider applicants working toward certification, supplemented by knowledge and experience in mental health/substance abuse recovery and must be able to serve as role model of recovery; or any equivalent combination of education and experience that provides the following knowledge, skills and abilities.
Licensure / Certification:
Maintain current CPR certification from the American Heart Association.
Skills / Ability:
Verbal and written fluency in English. Creole and Spanish desirable. Knowledge of community resources in order to assist with linkage to mental health treatment, services and other community resources. Ability to follow directions and communicate effectively. Ability to establish and maintain effective relationships with co-workers, judiciary, court administration and community agencies. Ability to utilize public transportation. Ability to use computer programs such as Microsoft Office Suites and Share Point. Ability to manage time effectively. Ability to apply excellent organizational skills.
POSITION RESPONSIBILITIES (THIS IS A NON-EXEMPT POSITION)
Maintains an active caseload of clients who need case management and supportive counseling on an ongoing basis.
Implements ongoing monitoring of clients’ needs, and evaluation of services.
Exercise and promotes a functional referral system with all departments within CHI system to ensure quality care.
Coordinates with various external resources to provide services for patients as needed.
Maintains records of services provided to clients and completes records for funding programs, assists in gathering and completes records for funding programs, assist in gathering and compiling information for internal and external reports.
Participates in patient/community education (internal and external) and home visits when necessary.
Communicates to supervisor/appropriate staff pertinent information.
Participates in Quality Assurance Program and continuing education as required.
Maintains all licenses and/or certificates as required for job.
Maintains 80% productivity monthly.
Reports to work on time and ready to work with minimal absenteeism.
Completes B&E (billing and encounter) forms within the same day of intervention.
Provides accurate and timely documentation in patient charts within 24 hrs of intervention.
Adheres to Confidentiality Policies and Procedures / HIPAA Regulations.
Performs other duties as assigned.
