Conducts needs assessments and keeps up-to-date on changes in health care technology to keep program material current. Being a Health Educator develops, coordinates, and disseminates educational resources and materials. May provide health education outreach and consultation. Additionally, Health Educator requires a bachelor's degree in a related area. Typically reports to a manager or head of a unit/department. The Health Educator work is generally independent and collaborative in nature. Contributes to moderately complex aspects of a project. Working 2 days in Silsbee and 3 days in Jasper each week, the Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. In addition, the CHW will provide social support and informal counseling, advocate for individuals and community health needs, and provide related services.
