Job Position: Registered Nurse Home Health Case Manager

Location: Illinois City, IL

Duration: 03 Months Contract

Shift: 08:00 AM - 04:30 PM

Position Summary:

Unit Coverage: Sauk Village, Chicago Heights, Beecher, Crete, Park Forest, Richton Park, University Park with overflow in Monee, Matteson, Olympia Fields

Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc. Patient Ratios: Case Manager: 20-30 patients Required Certifications: BLS Skills required: Wound care, including wound vacs, pleural drains, ostomy care; peripheral blood draws, central line care, IV administration through multiple devices including elastomeric and IV pumps, excellent assessment skills, experience with computer documentation
