- Provide direct patient care and case management services to assigned patients
- Conduct comprehensive assessments of patients' medical conditions and needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and evaluate patients' progress towards achieving health goals
- Coordinate and facilitate communication between patients, families, healthcare providers, and community resources
- Conduct regular patient visits to assess their needs and provide support and education
- Collaborate with physicians and other healthcare professionals to ensure optimal patient outcomes
- Utilize medical records and documentation systems to maintain accurate patient information
```Qualifications:```
- Current Registered Nurse (RN) license in the state of Kansas
- Minimum of 1 years of experience in acute care, or hospital setting
- Strong knowledge of discharge planning, primary care, and patient care coordination
- Familiarity with utilization management principles and processes
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Proficient in medical record documentation and electronic health record systems
- Solid understanding of human anatomy, physiology, and medical terminology
