    Klickitat Valley Health is a rural, critical-access hospital located in scenic Goldendale, Washington only two hours from Portland along the breathtaking Columbia River Gorge. KVH offers emergency and acute inpatient care and a full scope of ancillary services including General Surgery, Diagnostic Imaging, Laboratory, Respiratory Therapy, PT / OT, Behavioral Health, Dental, Pain Management, Optometry and Hospice. The hospital also owns a busy, well-established primary clinic that is located on hospital campus. Inpatient care is managed by 24/7 Hospitalist staff. KVH employs over 300 staff.

     

    GENERAL SUMMARY:

    The Hospice/Palliative Care Quality & Informatics Analyst is a dynamic and highly skilled professional who plays a vital role in optimizing clinical workflows, ensuring data integrity, and driving continuous quality improvement within our programs. This individual leverages their expertise in the Meditech Home Care Electronic Medical Record (EMR) system to provide technical support, conduct training sessions, and implement innovative solutions that enhance patient care. Serve as a key player in grant management, overseeing the preparation and delivery of comprehensive reports to the Washington Rural Palliative Care Initiative (WRPCI). Ensure an exceptional patient experience serving as a front line contact for patients who reach our team by phone.  Assist with resource management helping to coordinate site visits for providers. 

     

     

     

    Qualifications


    Minimum Education, Training & Experience

     

    Education / Licensure:

    • Associates degree required.

    • Bachelor's degree in Health Informatics, Nursing, Health Administration, or related field preferred.

    Experience:

     

    • 2-5 years of clinical experience in a healthcare setting, preferably in Hospice and/or Palliative Care.

    • Prior experience working with Electronic Medical Records, analyzing data

    • Prior experience working with Quality Improvement initiatives

    Skills/Knowledge/Abilities:

    • Ability to read, write, speak and understand English.

    • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, staff, and healthcare providers.

    • Demonstrated expertise in data analysis and quality improvement methodologies.

    • Advanced analytical and problem-solving skills.

    • Strong organizational and time management skills.

    • Proficient in Microsoft Office and Google Suites and database management software.

    • Proficiency with Electronic Medical Records (EMR), typing speed of at least 40 words/minute.

    • Ability to transform complex data into easy-to-understand reports, charts, graphs.  

    • Professional written and verbal communication skills 

    • Comprehensive knowledge of medical terminology and quality improvement principles.

    • Strong understanding of grant management principles and procedures.

    • Self-initiative and strong independent judgment abilities 

    • Ability to coordinate resources based on patient need oftentimes under stressful circumstances.

    • Report-writing capabilities strongly preferred.

     

    GENERAL RESPONSIBILITIES (but not limited to): 

     

    • Informatics:

      • Oversee the maintenance and optimization of the Meditech Home Care EMR system.

      • Develop expertise in system dictionaries, documentation interventions, and flowsheet construction.

      • Train staff on the use of Meditech Home Care and provide ongoing support.

      • Identify opportunities for system enhancements and implement solutions to improve efficiency and workflow.

      • Serve as a resource for staff on EMR-related inquiries and troubleshoot technical issues.

      • Proficient in the use of Meditech Home Care and other relevant EMR systems.

    • Quality Assurance:

      • Conduct systematic audits across both Hospice and Palliative Care programs, focusing on data analysis and quality improvement initiatives.

      • Review medical records to ensure completeness and accuracy of documentation.

      • Collect and analyze data to track trends, identify areas for improvement, and measure the effectiveness of interventions.

      • Prepare comprehensive reports summarizing quality improvement findings and recommendations.

    • Grant Management:

      • Manage all aspects of grant deliverables, including data compilation, report writing, and communication with WRPCI.

      • Develop and implement strategies to ensure grant compliance and maximize funding opportunities.

      • Track grant expenditures and prepare reports for grantors.

    • Data Management:

      • Manage statistical data, quality assurance reports, and conduct chart reviews.

      • Review patient referrals and manage on-time certification and recertification processes.

      • Ensure timely transmission of OASIS data to CMS.

      • Review documentation of patient encounters, track Chronic and Complex Chronic Care Management (CCM). minutes, and prepare detailed and timely reports for Patient Financial Services.

    • Additional Responsibilities:

      • Provide technical support for Remote Patient Monitoring (RPM) and monitor the dashboard for potential issues.

      • Respond to department inbound calls.  Assist patients with scheduling appointments and making transportation arrangements.

      • Provide information on community resources and ensure follow-up.

      • Attend and participate in ongoing training and team meetings.

      • Uphold all organizational policies, procedures, safety standards, and ensure regulatory compliance.

      • Carry out additional duties as assigned to support the department's needs.
