Intake Director - RN

Constellation Health Services is looking for a Registered Nurse Intake Director to oversee our Intake Team.  

Qualifications: 

• Licensed RN in the state of NY and your license must be in good standing. 
• Associate's Degree in Nursing; BSN preferred.
  
• Minimum of 2 years clinical nursing experience, previous management and home care experience preferred. 
• Knowledge of home health eligibility.
  
• Knowledge of workers compensation, no fault and insurance authorizations. 
• Knowledge of Face-to-Face requirements. 
• Working knowledge of general computer programs (ie: Microsoft Office, Smartsheets, EMRs). 

Responsibilities: 

• Responsible for the acceptance, processing, and coordination of all referrals for patient care services.
  
• Assures quality patient care by receiving and analyzing each intake request for clinical and financial appropriateness.
  
• Oversees processes for insurance verification and billing accuracy.
  
• Receives and processes intake data from referral sources adhering to compliance requirements of CMS and other payers. 
  
• Coordinates all patient services in coordination with the Director of Clinical Services and/or Case Managers and Clinical Supervisors. 
• Weekend and holiday rotation as needed. 
• Will perform other duties as assigned.
