Job Summary:
We're looking for a Director of Nursing (DON) to assist with planning, coordination, leading, controlling, and evaluating home health services. The DON is responsible for coordinating and liaising with relevant supervisory and affiliate Departments and committees, developing policies and guidelines, obtaining input from service personnel, and providing professional input to related administrative systems. The DON directs and supervises professional and auxiliary personnel providing client care services, performs marketing duties, fosters public relations, consults and plans with health care facilities on staffing needs, and communicates the concept of quality client care to the general community. The DON interprets service needs to the Administrator, acts as a client services administrative liaison, and is available during all hours of operation (8am-5pm Mon-Fri).
Job Conditions:
This role requires efficient management of deadlines in a dynamic work environment, with minimal lifting involved. The position primarily entails desk work, including sitting, standing, stooping, walking, and extensive telephone communication. Occasional travel by car or airplane is necessary for local, out-of-town, or state seminars, conferences, or meetings. Proficiency in reading 12-point or larger font and effective communication in English, both verbally and in writing, is essential. At times, the DON may need to provide direct patient care in the field, necessitating strong communication skills, driving to patient locations, offering physical assistance as required, familiarity with medical equipment, and access to patient medical records.
Equipment Operation:
The role requires proficiency in using a PC, calculator, multi-line telephone, and other office equipment. Additionally, the Director of Nursing (DON) may need to utilize an iPad and medical equipment while in the field.
Company Information:
The Director of Nursing (DON) has access to all client medical records, client financial accounts, personnel records, and company financial records, which are reviewable by all management staff.
Promotes Staff Development:
- Harnesses the skills of nursing and home attendant personnel in developing orientation and staff education programs.
- Identifies leadership potential and provides opportunities for leadership training and development.
- Encourages and supports personnel in their pursuit of ongoing self-improvement through formal education, educational meetings, and active involvement in professional and related organizations.
- Assists in conducting interviews and selecting home healthcare personnel.
Responsibilities include but not limited to:
- Participates with the Administrator and Executive Director in the formulation of a business plan, ie., strategic goals, resource allocation plan(s) and a yearly budget.
- Consults, plans, and works with the Administrator in policy formulation and programmatic development and implementation including assessing adequate and appropriate staffing.
- Coordinates with Administrator planning activities to develop administrative policies and practices for the Agency.
- Consults with supervisors and staff, individually or in groups, regarding clients or programs or service programs.
- Ensures field assessments of all personnel are completed to meet Medicare requirements.
- Directs the implementation of service goals and objectives.
- Develops standards that ensure safe and effective services to clients and families.
- Establishes job functions, qualifications, and job descriptions for each service staff (direct and supervisory).
- Involved in hiring process including resume reviewing and interviewing
- Performs 90-day and annual reviews of all nursing staff
- Establishes and maintains effective channels of communication.
- Keeps program personal up to date with state-of-the-art clinical information and practices staff development including orientation, in-service education, and continuing education.
- Maintains policy and procedure manuals and updates procedures and job descriptions, as policies and procedures change.
- Directs staff in performance of their duties.
- Oversees the implementation and evaluation of client care services inclusive of regulatory licensure and certification criteria and accreditation standards.
- Coordinates with other program areas and senior management, as appropriate, according to the organizational structure.
- Acts as a liaison with Staff Development for program areas.
- Assesses appropriate staff supervision during all service hours.
- Collaborates in the development of an effective and efficient client care documentation system, including statistical compilation, and analysis, relative to cost and staff service delivery.
- Leads general orientation for all new employees as well as facilities completion of all annual education requirements for all-staff..
- Keeps staff informed of Agency developments, needs, etc.; and encourages their participation in problem-solving, at the organization and community levels via email, phone, or monthly virtual all-staff meetings.
- Assists in the evaluation of community needs and plans programs and services to meet with identified needs.
- Submits formal written proposals to Administrator for all new programs or services (with complete cost/benefit analysis).
- Oversees all CQI, QAPI and utilization review activities.
- Assists in Agency evaluation activities.
- Participates in selective conferences and workshops to promote knowledge in community health trends, efficiency in job performance, and overall professional growth.
- Participates with other health, civic, education, and professional groups, as directed, by the Administrator and Executive Director.
- Serves on, and periodically reviews, policies relevant to the client care with the Governing Body.
- Assists in marketing the Agency and fostering community relations.
- Consults with physicians in matters relating to the client care services.
- Consults and plans with healthcare facilities regarding the staffing needs.
- Evaluates own job performance and utilization of resources in planning for professional growth.
- Participates in state and national organizations, meetings, seminars, workshops, and activities relating to the health professions and healthcare services, when appropriate.
- Makes patient and personnel assignments.
- Coordinates patient care.
- Coordinates referrals via Careport/Wellsky and faxed requests..
- Ensures that patient needs are continually assessed.
- Ensures the development, implementation, and updates of the individualized plan of care.
- Ensures all orders are signed and communicates with MD/MD office to collect unsigned orders in a timely manner
- Maintains compliance with all quarterly and annual required tasks and logs for home health agencies including but not limited to: emergency preparedness, QAPI, incident reports, adverse events and infection control
- Participates as a team during home health and Medicare surveys and creates and submits a detailed plan of corrections to address any deficits found during surveys as needed.
- Performs quarterly chart audits and addresses deficiciences to ensure clinical documentation of field-staff is in compliance.
- Adheres to HIPPA regulations to ensure confidentiality of all clinical records.

Required Qualifications:
- Licensed Registered Nurse licensed in the state of Virginia.
- A minimum of 5 years of RN experience.
- A minimum of 1 year of home health experience.
- Minimum of 1 year of nursing management experience, involving hiring, coaching, performance management, scheduling, and daily operations oversight.
- Ability to pass a background check
- CPR and First Aid Certification
- Experience and understanding of OASIS Data Set and 485 POC
- Understanding of Medicare regulations and how they apply to the home health setting
- Agreeableness to have time split between the office and in the field performing home visits until more nurses are hired and on-boarded
- Agreeableness to be on-call for emergencies and complete the on-call log.
- Demonstrated knowledge of nursing practices aligned with regulatory requirements,
- Ability to communicate with the MD and acquire signed orders and communicate concerns
- Understanding of infection control practices and disease prevention.
- Ability to delegate tasks based on skill levels and compliance with regulations.
- Strong organizational, communication skills and ability to manage multiple priorities effectively.
- Critical thinking, clinical judgment, and decision-making abilities required.
- Proficiency in electronic medical records, Microsoft Office suite and ability to adapt to new applications.
- Valid driver's license and reliable transportation with current automobile insurance.
- Compliance with Jobeka Health Care Services LLC's policy requiring full vaccination against COVID-19. This includes receiving a minimum of one dose of a one-dose vaccine or two doses of a two-dose vaccine.
Preferred Qualifications:
- Previous experience with Alora or a Home HealthCare EMR system is strongly preferred.
- Capable of adapting and acquiring new skills as necessary, with a commitment to collaborative teamwork.
- Understanding of checking eligibility and performing prior authorizations a bonus
- Willingness to perform marketing tasks visits to local hospitals and offices as assigned
- Preferred experience in staff development, training, or clinical education.
- Preferred experience in analyzing clinical performance data, quality improvement, and risk assessment.
