Health Information Services Director directs and oversees all policies and procedures related to healthcare information management. Responsible for ensuring accuracy and efficiency in the medical records and transcription departments. The Health Information Services Director manages a departmental sub-function within a broader departmental function.     SUMMARY:

    The Support Services Supervisor is responsible for the provision and coordination of Targeted Care Coordination and Case Management services to clients. This position supervises a team of   professionals and support staff. The Targeted Care Coordination & BH Case Management department will assist clients in making choices for the full and appropriate   use of available services through the use of consultation, service linkage, advocacy, and information about community, county, and state resources. This role will also monitor adherence to regulations and the accurate and prompt completion of all reporting, billing, and other required paperwork.

    Support Services Supervisor is a full time position with a dual supervisory and clinical role. Some of their work hours will be blocked for supervisory activities and the rest of the time will be providing direct clinical services to child and adult patients. Starting annual salary of $57,000 DOE

     

    MINIMUM   QUALIFICATIONS:

    Master's degree in Professional Counseling, Social Work, or an equivalent field. Current State of Idaho Clinical Social Worker/Professional Counselor license in good standing required or license eligible. Knowledge of care coordination and case management preferred. 

     

    KNOWLEDGE,   SKILLS, AND ABILITIES REQUIRED:

    • Knowledge and understanding of evidence based masters level mental health counseling principles, methods, procedures, and standards for adults. 
    • Knowledge and understanding of patient support services and processes as it relates to care coordination and case management.
    • Interviewing and psychological/developmental evaluation skills. 
    • Skill in preparing and maintaining patient records. 
    • Knowledge of community mental health and behavioral health resources. 
    • Knowledge of clinical operations and procedures for age groups being treated at our clinics. 
    • Ability to create, compose, and edit written materials. 
    • Ability to make clinical and procedural decisions and judgments. 
    • Ability to maintain emotional stability to cope with human suffering, emergencies, and other stresses.        
    • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. 
    • Ability to evaluate the progress of support service programs and to make individual modifications. 
    • Knowledge of crisis intervention techniques. 
    • Ability to observe, assess, and record symptoms, reactions, and progress. 
    • Knowledge of legal and ethical issues related to patients' rights. 
    • Knowledge of community support services and funding agencies. 
    • Knowledge of community health care and vocational services. 
    • Ability to problem solve effectively with multidisciplinary team.

     

    DESCRIPTION OF DUTIES:

    Position requires a) provision of intensive, integrated professional mental health services and treatment planning in a clinical and/or outpatient environment;   b) provision of professional leadership in the determination of appropriate therapeutic approaches and methods; c) planning and implementing   outreach/education programs, workshops, and activities; d) supervising   clinical activities of therapists, for identified program; and e) establishment and maintenance of case files, supervision files, and related documents for clinical and legal purposes.

    1. Collaborates with the BH Director and therapist(s) to determine continuum of care for program participants.
    2. Directs BH Care Coordinators in the day-to-day provision of family-focused, trauma-informed support services.
    3. Collaborates with BH Director to complete annual employee reviews for BH Care Coordinators.
    4. Oversees Peer Review process for BH Care Coordinators, reporting summary and concerns to the BH Director.
    5. Reviews and signs TCC & case management documentation as required by Medicaid protocols.
    6. Plans and facilitates community outreach and education activities, as appropriate to the position. May serve as a liaison and        representative to community organizations and schools. 
    7. As appropriate to the position, performs various administrative functions such as identifying program barriers and problem solving with staff and BH Director to find resolutions. May propose changes to program policies and procedures to BH Director. 
    8. May provide formal teaching, consultation, and in-service training to relevant professionals. In addition, helps with the immediate and proper handling of referrals of a variety of matters.
    9. Provides counseling, therapy, and/or psychotherapy to patients and families as appropriate to the position. Prepares treatment plans, discharge plans, and follow-up care programs. Provides therapeutic crisis intervention and emergency services as required. 
    10. Collects data about patients through interview, case history, screenings, and/or observational techniques. Evaluates data to identify causes of problems and to determine proper level of care or referral to other specialists. 
    11. Consults with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework. Refers clients to appropriate service agencies as required. 
    12. Provides and/or arranges for BH interventions as appropriate for patients in a crisis condition and for those with serious disturbance problems. 
    13. Refers clients to appropriate social service agencies for financial assistance and other required services. 
    14. Follows up to determine reliability of treatment used, and changes method and degree of therapy when indicated. 
    15. Establishes and maintains case files, referrals, and other related documents for the treatment of clients; prepares related documentation for civil commitment hearings or other legal proceedings. Keeps abreast of patient data to ensure appropriate treatment and care are delivered. 
    16. Consults with other legal and treatment agencies and individuals in relation to patient/client records, rights, and responsibilities. 
    17. Performs miscellaneous job-related duties as assigned.
