    JOB SUMMARY

    The job is responsible for key leadership with all aspects of administering SDoH community health initiatives in New York and more specifically Buffalo, to include but not limited to crafting and implementing SDOH strategies, building cross-sectoral collaborations, overseeing research and data-driven interventions, and promoting health equity. Interfacing regularly with various internal departments, this position will implement strategic objectives into actionable programs, supporting evaluation frameworks to determine program effectiveness and offering essential guidance to the communities we serve. Operating within a matrixed structure, this position requires expert navigation of indirect management and collaboration across multifaceted teams throughout the enterprise. Working hand-in-hand with the SVP and VP Social Determinants of Health (SDOH), this position will drive the development and realization of the CBO engagement strategy, addressing the nuanced SDOH needs of Highmark Health. They will establish close ties with key community benefit organizations (CBOs) throughout Highmark's domain, fostering integration of CBO services with core departments, business units, and internal stakeholders to amplify health outcomes. This job is pivotal in driving and executing the overall SDoH strategy to achieve positive community health outcomes.

    ESSENTIAL RESPONSIBILITIES

    • Perform management responsibilities to include, but are not limited to: involved in hiring and termination decisions, coaching and development, rewards and recognition, performance management and staff productivity. Plan, organize, staff, direct and control the day-to-day operations of the department; develop and implement policies and programs as necessary; may have budgetary responsibility and authority.
    • The incumbent will be provide design and operational support and will be instrumental in driving strategy for initiatives to make an impact on community health outcomes. Monitor and evaluate emerging SDOH trends and integrate relevant insights into the organization's strategies. Develop and implement strategic plans related to SDOH, ensuring alignment with the organization's overall mission and objectives.
    • The incumbent will lead program development and management. Design, implement, and oversee programs and initiatives aimed at addressing social determinants and their impact on health. Establish metrics and evaluation methods to assess program effectiveness and adapt strategies accordingly.
    • The incumbent will oversee and contribute to evaluation of SDOH projects and initiatives.
    • Translate evaluation findings into actionable strategies and recommendations for the organization and its stakeholders. Collaborate across the Department and Enterprise Data Analytics to establish metrics and evaluation methods to assess program effectiveness and adapt strategies accordingly.
    • The incumbent will develop and promote SDoH education and training. Develop and facilitate training programs for staff and partners to build understanding and capacity around SDOH. Promote public awareness of SDOH and their impact on health outcomes.
    • The incumbent will support program sustainability. Identify and pursue opportunities to increase extramural funding to support SDOH community health initiatives research activities. Support grant proposals, secure partnerships, and engage with potential funders to expand financial resources for community based SDoH initiatives.
    • The incumbent will collaborate with internal and external stakeholders. Collaborate with internal departments, community organizations, policymakers, and other stakeholders to create comprehensive approaches to SDOH. Serve as an organizational spokesperson on SDOH matters, representing the organization at conferences, public events, and with media.
    • Other duties as assigned or requested.

    EXPERIENCE

    Required

    • 5 years of experience in management or leadership role
    • 7 years of experience in community health, population health, program development, project implementation, engagement, or a related field
    • 3 years of experience in qualitative and/or quantitative evaluation and/or research

    Preferred

    • 8-10 years of experience working in New York and/or Buffalo on community health, population health, program development, project implementation, engagement
    • 3 years of experience in contributing and/or securing extramural funding through grants, partnerships, or other funding mechanisms

    SKILLS

    • Strong knowledge of social determinants of health and their impact on population health outcomes.
    • Ability to collaborate with community leaders, healthcare professionals, and local organizations to identify priority health issues affecting the target communities.
    • Capability to develop and implement evidence-based health initiatives that address identified health inequities and align with the community's needs and resources.
    • General knowledge of health evaluation.
    • Excellent analytical and problem-solving skills, with the ability to translate data into actionable insights.
    • Strong leadership and project management abilities.
    • Excellent communication and interpersonal skills, with the ability to collaborate effectively with diverse stakeholders.
    • Ability to work across a highly matrixed organization

    EDUCATION

    Required

    • Bachelor’s degree in Public health, population health, health policy, or related field OR relevant experience and/or education as determined by the company in lieu of bachelor's degree.


    Preferred

    • Master's degree in Public health, population health, health policy, or related field
    • Doctorate degree in Public health, population health, health policy, or related field

    Pay Range Minimum:

    $108,000.00

    Pay Range Maximum:

    $199,800.00

    Base pay is determined by a variety of factors including a candidate’s qualifications, experience, and expected contributions, as well as internal peer equity, market, and business considerations. The displayed salary range does not reflect any geographic differential Highmark may apply for certain locations based upon comparative markets.
