Job Summary:
The RN Case Manager works in collaboration with physicians and other health care professionals to provide comprehensive health assessment, treatment and follow up evaluation for patients.
- Verifies and implements physician/other licensed independent practitioners’ orders.
- Assess and chart observations of the patient’s condition upon each visit.
- Complete evaluation tasks, including reviewing medications and vital signs.
- Administer Physician-prescribed medication.
- Dress or redress wounds and assess healing progress.
- Provide education to patients and families on proper home health care procedures and strategies as well as education on disease processed and when to contact a health professional.
- Coordinate with physicians, physical therapists, occupational therapists and other individuals involved in the patient’s care plan.
- Conducts assessments and identifies potential discharge needs.
- Participates in discharge planning and coordinates identified discharge needs referring to community resources as appropriate.
- Documents accurately and completely pertinent assessments, interventions, and outcomes for patients in accordance to documentation standards, policies, procedures and/or guidelines.
- Effectively manages resource utilization to ensure efficiency and appropriate reimbursement.
- Integrates clinical, financial, ethical, compliance and other variables in decision making.
Requirements:
Required
- Active RN license in the state of Ohio
- Current AHA BLS certification
- Valid Driver’s License
- Proof of current car insurance
Preferred
- Bachelor’s degree in Nursing or related field
- Certification in Case Management
