Home Care Aide Coordinator schedules workforce needs of Home Care Aides. Tracks aide services and collects and maintains time reports. Being a Home Care Aide Coordinator typically requires a high school diploma. Typically reports to supervisor or manager. Working team member that may validate or coordinate the work of others on a support team. Suggests improvements to process, is a knowledge resource for other team members. Overview
The Hospice Spiritual Care Coordinator is responsible for the provision of spiritual care services to patients, families and caregivers of Signature Hospice, either directly or through coordination of care with other spiritual counselors.

Responsibilities
- Facilitate the development of the individualized plan of care to meet identified spiritual needs.
- Assure spiritual assessment of patients, families and caregivers are appropriate and services are rendered in a timely manner.
- Provide direct spiritual care to patients, families and caregivers.
- Provide for funeral of memorial services.
- Recruit community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient needs by developing community contacts and offering education through congregations about hospice care.
- Assist in supervision of spiritual care volunteers.
Qualifications / Skillsets
- Must possess a Bachelor of Divinity (BD) or Master of Divinity (MDiv) degree from an accredited seminary or school of theology, or equivalent theological degree.
- Experience working with death and dying, and the spiritual needs of the terminally ill, their family and caregivers.
- Must have a valid driver’s license and current automobile insurance.
- Understanding and commitment to the philosophy of hospice concept.
