JustCare HomeCare is seeking to hire an energetic Home Care Coordinator. We give personal attention to our customers’ needs and expectations and we set our standards high.
Responsibilities:
Responsible for scheduling and assigning Home Health Aide to meet patient needs
Ensure all Patients are receiving services
Ensure that services are provided as prescribed by the client's Plan of Care.
Assign appropriate Home Health Aide's (HHA) with patient's while providing HHA's with travel directions.
Manage Schedules of Home Health Aides and Patients.
Document all changes and occurrences for Patients and Aides on a daily basis
Communicate with patients and aides to ensure they satisfied with care provided
Keep tabs health status with the patient through bi-weekly progress calls
Prebilling and working on Call Dashboard on a daily basis
HHAEXCHANGE knowledge preferred
Bilingual (English, Russian) preferred
Other related duties as assigned
