    Primary Purpose
    The primary purpose of this position is to direct the overall operations of a Senior Care Partners P.A.C.E. center and the interdisciplinary team(s) (IDTs). Primary duties include developing and maintaining a strong and unified IDT and ensuring the delivery of exceptional participant care. The Director of PACE IDT Operations will fully endorse, promote, and engage the Interdisciplinary Team with essential foundational PACE concepts.
    Duties and Responsibilities
    • Responsible for creating an inspiring team environment where direct reports feel empowered and equipped to take ownership of their areas of responsibility, while being intentionally developed, appropriately challenged, and regularly recognized.
    • Manages operations and facilities effective interdepartmental communication and teamwork.
    • Ensures that the site is in compliance with all Centers for Medicare and Medicaid Services (CMMS) and State Administering Agency (SAA) guidelines and regulations.
    • Ensures emergencies or unusual incidents concerning staff or participants are handled timely and appropriately.
    • Supports and participates in activities associated with marketing and community engagement.
    • Maintains a key role in identifying on-going training and education needs for Senior Care Partners PACE staff.
    • Ensures proper staffing levels and work schedules for center operations.
    • Participates in setting and achieving organizational benchmarks and performance goals.
    • Prepares periodic reports to review staff performance as related to planning, caseload allocation, and quality of work, procedures, and participant care and outcomes.
    • Regularly attend assigned committees, and actively participates in Quality Improvement activities of the organization.
    • Participates in the participant grievance and appeals process.
    • Assures organizational adherence to the Senior Care Partners PACE mission and vision and assists guiding long-range objectives.
    • Works collaboratively with Associate Director team to drive organizational goals and successes.
    • Assists with development of an overall business plan and budget for the Senior Care Partners PACE centers (following approved format) and is responsible for management of the plan and budget.
    • Develops and implements written policies and procedures governing the operation of Senior Care Partners PACE.
    • Ensures accurate and appropriate documentation in the medical record and participant changes are communicated to the IDT in an efficient manner.
    • Monitors resources closely and implements initiatives to ensure operational efficiency, staff productivity, and cost effectiveness of services.
    • Empowers employees and teammates by sharing responsibility, setting, and communicating attainable expectations, objectives, and goals for others, and supporting a spirit of commitment, ownership, and teamwork.
    • Effectively communicates, in written and verbal form, with internal staff and the broader community.
    • Must care, support, and collaborate with everyone involved in participant's care including the participant’s family using the the patient-family centered care approach.
    • Other duties as assigned.
    Qualifications
    • Bachelor's Degree in a health field with a major in management or administration, or any combination of experience, education, or training which would provide the knowledge required by this position.
    • Licensure as an RN/MSN in the State of Michigan preferred.
    • Must have a minimum of 5 years of experience working in a health or human services, managed care, home care, or long-term care setting.
    • Must have a minimum of 3 years in a leadership role.
    • Minimum of one year of experience working as part of a multidisciplinary or interdisciplinary health care team.
    • Must have the ability to get along harmoniously with others in a team environment.
    • Must possess the ability to communicate effectively with all stakeholders.
    • Must possess the ability to plan, organize, develop, implement, and interpret goals, objectives, policies, and procedures necessary for providing quality care.
    • Proficiency with MS Office Suite, Word, Outlook, PowerPoint, & Excel.
    • Minimum of one year of experience working with frail, elderly, or long-term care population.
    Minimum Working Requirements
    • Must have reliable transportation, current valid Michigan driver’s license and car insurance.
    • Must be 18 years of age or older.
    • Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B.
