As a MEDICAL SOCIAL WORKER at Hospice of Warren County, you will:
· Assess the emotional, psychological, and social needs of patients and their families.
· Collaborate with the interdisciplinary team of doctors, nurses, chaplains, dieticians, counselors to develop a plan of care to address needs related to terminal diagnosis.
· Assist patient, family, and staff in utilizing community resources to meet defined needs.
· Provide counseling, support, and resources to address emotional and practical issues.
· Attend and collaborate with a team of dedicated professionals, including our medical director, nurses, chaplains, bereavement & volunteer coordinators.
· Facilitate communication between patients, their families, and the healthcare team.
§ Maintain detailed records and documentation.
§ Assess patient/family ability to meet financial obligations and offer appropriate resources and activate as indicated.
§ Evaluate patient’s insurance coverage and options related to hospice benefit.
§ Assist in discharge planning from hospice service when care is no longer appropriate.
§ Master’s level candidates will provide monthly supervision to non-masters SW staff.
QUALIFCATIONS: To thrive in this role, you should have:
· A Bachelor's or Master’s degree in social work. (Master’s level preferred).
· Licensure as required by Pennsylvania state regulations.
