HOSPICE SOCIAL WORKER

Comfort 1 Hospice is seeking a Social Worker to join our dedicated Hospice Team in South Bend, Indiana. As a member of the Comfort 1 Hospice interdisciplinary team, you will provide psycho-social assessments, social service counseling, needs assessments, and guidance to patients and family members. Additionally, the Social Worker will assist the Physician and other members of the Hospice Team in understanding significant social/mental stress and/or disorder that increases the symptoms related to terminal illness. Your visits will be between skilled nursing, independent and assisted living facilities and private residences.

Responsibilities:

As a Hospice Licensed Medical (LMSW) or Licensed Clinical Social Worker (LCSW), you will:

• Provide wide range of psychosocial services to terminally ill patients and their families.
• Responsible for contacting patient, family, and caregiver during the admission process.
• Psychosocial must become part of patient’s chart.
• Regularly assess the patient and family psychosocial needs.
• Regularly assess the pre-bereavement/anticipatory grief needs.
• Implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
• Regularly re-evaluate the patient’s and family’s psychosocial needs.
• Participate in weekly interdisciplinary team meetings.
• Assist with making funeral arrangements if requested.
• Provide counseling during the process of the terminal illness and at the time of death if requested.

Qualifications:

• A required Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education.
• Current Licensed Social Work Indiana state license (LSW).
• Valid Driver’s license and current auto insurance is required.
• Minimum of one year social work experience in a hospice healthcare setting or social services agency
• Knowledge of and ability to obtain community resources; information and referral, state/federal financial programs, Medicare, Medicaid eligibility requirement and insurance.
• Understand Hospice philosophy, principles of death/dying.
• Strong organizational and communication skills.

Experience:

• Minimum of one year social work experience in a hospice healthcare setting or social services agency

License:

• Required Master of Social Work (MSW)
• Current Social Work Indiana state license (LSW).
