Hospice Director directs, supervises, and governs the hospice program including inpatient care, home care and bereavement follow-up. Administers and maintains quality assurance, environmental, and infection control policies in accordance with facility objectives. Being a Hospice Director prepares reports for hospital administration on activities of the facility operation. Develops hospice related educational and informational programs. Additionally, Hospice Director requires a bachelor's degree of nursing. Typically reports to top management. The Hospice Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be a Hospice Director typically requires 3+ years of managerial experience.     Summary:Provides nursing care under the supervision of an RN by performing the following duties:

    • This is a Full time position.
    • $23-$26/hour

    Duties and Responsibilities:

    • Demonstrates expertise as a subject matter expert in death, dying, and grief.*
    • Responsible for providing quality end of life care. Striving for a 90% benchmark on all data points.*
    • Demonstrates compassion in all interactions representing the agency.*
    • Acts professionally in all interactions.*
    • Acquires and maintains current knowledge of hospice nursing practices.*
    • Meets minimum productivity standards.*
    • Follows the plan of care as established by the interdisciplinary team and under the supervision of a registered nurse.*
    • Provides direct nursing care within the scope of LPN practice.*
    • Provides education to the patient/family as indicated in the plan of care.*
    • Observes and reports changes in patient/family condition to the primary nurse.*
    • Provides emotional support to the patient and family throughout the dying process.*
    • Maintains professional boundaries at all times.*
    • Documents accurately and submits notes on a daily basis.*
    • Demonstrates compliance with documentation requirements as evidenced by results on quality audits.*
    • Participates regularly in assigned PIPs.*
    • Supports the organizations mission, vision, and values.*
    • Demonstrates a commitment to the hospice philosophy of care.*
    • Demonstrates sensitivity to and an understanding of death, dying, and bereavement.*
    • Demonstrates ethical conduct and follows practices out lined in the corporate compliance plan.*
    • Demonstrates knowledge and understanding of agency policy and procedures outlined in the employee hand book.*
    • Complies with the health and safety requirements of federal, state, and local laws.*
    • Responsible for being a member of the Hospice Interdisciplinary group that assists group members in understanding the significant spiritual and religious factors related to the care of the terminally ill patients and families.*
    • Attends and participates in continuing education for professional development.*
    • Respects and protects the confidentiality of all agency information.*
    • Maintains flexibility in assignments to meet agency needs and accepts other duties as appropriate.*

    Language Ability:

    Ability to add, subtract, multiply, and divide.

    Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

    Computer Skills:

    To perform this job successfully, an individual should have knowledge of Microsoft Word, Google Docs, Microsoft Excel, Google Sheets, and electronic medical records.

    Education/Experience:

    Associate's degree (A.A./A.S.) or equivalent from two-year college or technical school and one to two years related experience and/or training; or equivalent combination of education and experience.

    Certificates and Licenses:

    • Current New York State LRN license
    • Current New York State driver’s license

    Equipment:

    • Hoyer lift
    • Catheter – straight or foley
    • Oxygen concentrator
