Hospice Director directs, supervises, and governs the hospice program including inpatient care, home care and bereavement follow-up. Administers and maintains quality assurance, environmental, and infection control policies in accordance with facility objectives. Being a Hospice Director prepares reports for hospital administration on activities of the facility operation. Develops hospice related educational and informational programs. Additionally, Hospice Director requires a bachelor's degree of nursing. Typically reports to top management. The Hospice Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be a Hospice Director typically requires 3+ years of managerial experience.     This position will have director duties over Waterloo and Cedar Rapids, IA

    This position has responsibility to lead, direct and coordinate clinical, financial and business operations of Hospice within a specified region as assigned by VP of Hospice. This position ensures the delivery of high quality, cost effective clinical care for all hospice patients. Works in conjunction with regional clinical leaders to facilitate implementation of action plans related to strategies to ensure operational and financial success. Ensures coordinated and harmonious working relationships among assigned departments and between operations and other UPaH departments, other UnityPoint Health affiliates, and the communities served by UPaH.

    Responsibilities

    Key Accountability: Clinical and Operational Excellence - 30%

    • Evaluates current clinical practices within hospice for areas that may need improvement. Establishes improvement plans and expectations and ensures execution of improvement initiatives.
    • Engages staff in the means to gain efficiency while improving patient outcomes through innovation.
    • Works with other UnityPoint at Home leaders in the development of objectives, standards of care, policies and procedures affecting the programs of the organization.
    • Implements UnityPoint at Home standard record systems, statistical reporting systems, and program documentation, planning and evaluation.
    • Communicates with Vice President, Hospice regularly to review service issues and opportunities for improvement to ensure the delivery of high-quality service to all internal and external customers.
    • Ensures quality assurance and performance improvement initiatives are appropriate and effective and implemented within the region to ensure exceptional quality performance in areas including, but not limited to, CAHPS outcomes, patient experience, hospitalization utilization, patient safety measures, Hospice Value Based Purchasing, and workplace violence programs.
    • Creates and builds engagement of hospice leaders by building productive relationships that include face-to-face visits with each leader across all regional home health offices.
    • Creates and builds engagement with clinical team by building productive relationships that include regular time spent in all hospice locations within the region and leader-patient rounding via ride-along visits with clinicians and follow-up phone calls with patients.
    • Consistently participates in all hospice leadership meetings and ensures timely and accurate cascade of relevant information to clinical team members.
    • Creates a culture of collaboration, innovation, teamwork, and professional practice, consistent with the mission, vision and values of UnityPoint at Home.
    • Consistently serves as “first site” for innovative program implementation by cultivating an innovative and collaborate culture throughout the region and consistently executing on new initiatives as directed.

    Key Accountability: Employee Management – 30%

    • Ensures effective and efficient operations by optimizing performance and continually improving quality of care in a vastly changing health care environment.
    • Establishes effective mechanisms of communication with staff and leadership team to foster an environment of openness, trust, teamwork and professional development.
    • Establishes a culture of accountability throughout the region by ensuring all team members understand expectations and expected performance is consistently measured.
    • Demonstrates the skills identified in UPH “Leadership Essentials” in day-to-day interactions with all team members

    Key Accountability: Financial Management – 30%

    • Ensures hospice operations are efficient and effective by implementing and monitoring key financial metrics, including but not limited to, productivity, case mix, patient volume, referral management, clinical caseload management, home health value-based purchasing performance, and expense management.
    • Collaborates monthly with finance team to identify opportunities for improvement, develops plans for correction, and ensure execution of correction plan elements.
    • Collaborates with finance team and Vice President, Hospice to develop annual budget and long-term forecast for the region.

    Basic UPH Performance Criteria - 10%

    • Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
    • Demonstrates ability to meet business needs of department with regular, reliable attendance.
    • Employee maintains current licenses and/or certifications required for the position.
    • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
    • Completes all annual education and competency requirements within the calendar year.
    • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

    Qualifications

    Education: Graduate of accredited BSN nursing program. Preferred, but not required; MSN, MHA, other graduate-level degree in a healthcare-related field

    Experience: 2-3 years hospice or home care leadership experience. Perferred, but not required; 3-5 years hospice or home care leadership experience

    License(s)/Certification(s): Valid driver’s license in state of residence. Current license in good standing to practices as an RN in state where employee resides.

    Knowledge/Skills/Abilities:

    • Writes, reads, comprehends and speaks fluent English
    • Basic to intermediate computer knowledge using Microsoft Office programs
