Hospice Supervisor manages the resources, operations, and staff of hospice care programs. Works with families and patients to develop optimal care plans. Being a Hospice Supervisor coordinates the delivery of a patient/family assessment process and care plan development. Oversees interdisciplinary teams of medical doctors, nursing staff, and other care providers inside and outside the facility. Additionally, Hospice Supervisor maintains an up-to-date knowledge of laws and regulations of federal, state, and regulatory bodies relative to hospice care. May require a bachelor's degree in nursing, social work or equivalent. Typically reports to a manager or head of a unit/department. The Hospice Supervisor manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Hospice Supervisor typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required.     • Provides personal care activities of daily living such as bathing, shampooing, oral hygiene, skin care/nail care, shaving, and dressing.
    • Communicates changes in patient status to the supervising Registered Nurse (RN) in a timely manner.
    • Reports patient complaints to the RN
    • Creates and submits legible, accurate and timely documentation.
    • Performs duties in compliance with agency policies and procedures and local, state, and federal regulations.
    • Demonstrates attention to details for patient's safety and safety of their home environment.
    • Reports unsafe conditions in the home.
    • Practices effective infection control and adheres to infection control policies and procedures.
    • Practices safe lifting and transferring of patients.
    • Communicates clearly and positively with patients, families, team members and others.
    • Displays a caring, patient’s first behavior.
    • Keeps patients and/or family informed regarding visit time and schedule changes.
    • Attends in-service programs to meet compliance requirements
    • Maintains patient confidentiality in compliance with HIPAA

    Basic Qualifications

    • Must be a Certified Nursing Assistant (CNA) and/or possess current Home Health Aide registrar in the state of Indiana.
    • Must have high school diploma or equivalent
    • Must possess valid driver’s license and automobile insurance in the state of Indiana
    • Must be able to provide own transportation to assignments.
    • Ability to work independently
    • Ability to work in and support a team environment.
