Hospice Supervisor manages the resources, operations, and staff of hospice care programs. Works with families and patients to develop optimal care plans. Being a Hospice Supervisor coordinates the delivery of a patient/family assessment process and care plan development. Oversees interdisciplinary teams of medical doctors, nursing staff, and other care providers inside and outside the facility. Additionally, Hospice Supervisor maintains an up-to-date knowledge of laws and regulations of federal, state, and regulatory bodies relative to hospice care. May require a bachelor's degree in nursing, social work or equivalent. Typically reports to a manager or head of a unit/department. The Hospice Supervisor manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Hospice Supervisor typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required. Caring Circle Hospice at Home is a 16-bed inpatient hospice residence to serve patients and families needing this environment. We specialize in providing end of life care and symptom management to individuals with a life expectancy of six months or less. We serve individuals in their home, nursing facilities, and in the hospital. Scope of work
Offer customer and patient focused care that always exceeds expectations. This position provides for seamless continuity of nursing care between the regular business hours and after hours. This care may be in the absence of or as an adjunct to the RN Case Manager. Visits are made to hospice patients at home, facility, and hospital locations to manage emergent symptoms. Provide support to caregivers at the time of death. Demonstrate personal accountability for your own continuous growth and development. Seek appropriate opportunities to enhance competency and improve performance.
The RN, under the ANA scope of practice utilizes the framework of the nursing process to provide professional nursing care by incorporating patient-family centered care principles to assess, plan, implement, evaluate, and discharge. The RN provides quality patient care under the direction of the Nurse Leader according to Corewell Health policies and procedures and recognizes the impact of nursing care and patient/family partnerships on patient outcomes. The RN partners with and utilizes patient/family input and goals in planning plan of care and develops teaching strategies for patient/family; documents education and learning appropriately in health record.
