Hospice Supervisor jobs in Monroe, LA
Hospice Supervisor manages the resources, operations, and staff of hospice care programs. Works with families and patients to develop optimal care plans. Being a Hospice Supervisor coordinates the delivery of a patient/family assessment process and care plan development. Oversees interdisciplinary teams of medical doctors, nursing staff, and other care providers inside and outside the facility. Additionally, Hospice Supervisor maintains an up-to-date knowledge of laws and regulations of federal, state, and regulatory bodies relative to hospice care. May require a bachelor's degree in nursing, social work or equivalent. Typically reports to a manager or head of a unit/department. The Hospice Supervisor manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Hospice Supervisor typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required. Job Overview:
We are seeking a dedicated Registered Nurse Case Manager to join our Hospice Team. The ideal candidate will have 2 years Hospice experience. This position offers an opportunity to make a meaningful impact on patients and their families at a very meaningful time in their lives.
Responsibilities:
- Conduct patient assessments and develop individualized care plans
- Coordinate and collaborate with healthcare providers to ensure comprehensive care for patients
- Monitor patient progress and update treatment plans as needed
- Educate patients and families on health management strategies
- Document all interactions and interventions accurately in the patient's medical record
- Advocate for patients to receive appropriate services and resourcesQualifications:
- Valid RN license in the state of practice
- Minimum of 2 years of nursing experience in Hospice
- Proficiency in medical terminology and patient assessment techniques
- Strong communication and interpersonal skills
- Knowledge of managed care principles and practicesJob Type: Full-time
Pay: $69,680.00 - $72,800.00 per year
