QUALIFICATIONS

• Current state of Michigan Registered Nurse License and 5 years nursing and teaching/or, equivalent experience. BSN preferred, and BLS required.
• Broad knowledge of all aspects of home health care and hospice, with a minimum of two years’ experience.
• Certified hospice and palliative care preferred.
• Proficient in Clinical Technology and resources.
• Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

SUMMARY OF JOB RESPONSIBILITIES

The Home Care Supervisor provides professional development and education to the clinical staff to provide and improve the skills and knowledge needed to give the highest level of care to patients. The Home Care Supervisor provides field supervision of clinical staff and gives feedback to staff and management as to findings and creates improvement plans for any identified deficiencies or areas in need of improvement. The Hospice Home Care Supervisor participates in a variety of quality improvement projects and develops and leads initiatives to improve staff performance through education and training. Ensure clinical staff supervised meet regulatory requirements for licensure and clinical competencies.

Salary: From $80,000.00 per year
