Hospice Supervisor manages the resources, operations, and staff of hospice care programs. Works with families and patients to develop optimal care plans. Being a Hospice Supervisor coordinates the delivery of a patient/family assessment process and care plan development. Oversees interdisciplinary teams of medical doctors, nursing staff, and other care providers inside and outside the facility. Additionally, Hospice Supervisor maintains an up-to-date knowledge of laws and regulations of federal, state, and regulatory bodies relative to hospice care. May require a bachelor's degree in nursing, social work or equivalent. Typically reports to a manager or head of a unit/department. The Hospice Supervisor manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Hospice Supervisor typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required.     Job Description Summary

    The Performance Improvement RN/Nursing Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Performance improvement/Nursing Supervisor is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.

    Essential Job Functions/Responsibilities

    1. Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with MEDICAL DIRECTOR regarding any questions about an individual’s eligibility for services.

    2. Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.

    3. Reviews patient’s medical diagnosis, prognosis, medications, procedures and clinical course.

    4. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.

    5. Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.

    6. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director/Director of Patient Care Services and hospice personnel.

    7. Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new employees.

    8. Responsible for the implementation and monitoring of the organization’s quality assessment performance improvement (QAPI) program.

    a. Ensures that there are processes in place to monitor and measure all activities related to performance improvement.

    b. Provides direction and coordination of QAPI activities utilizing continuous quality improvement principles and methodologies.

    c. Coordinates activities related to clinical record review for data collection.

    d. Compiles, trends and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction. Data is analyzed and compared internally and externally with other sources when available.

    e. Works collaboratively with nursing supervisors to assure documentation is complete and consistent with care and reflects legal requirements.

    f. Member of the Patient Grievance Committee collaborates with management staff in follow-up of concerns expressed by patients, families, and physicians.

    g. Establishes a process model for improvement and provides education and training to organization personnel on the performance improvement plan, and team development.

    9. Assists Clinical Director/Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of inservice and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.

    10. Complies with accepted professional standards and principles.

    11. Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs. Participates in public relations and community activities that promote the organization’s role as an effective member of the hospice and health care delivery system. Promotes customer service orientation to all hospice personnel.

    12.Participates actively in quality assessment performance improvement program. Assists with the evaluation of organization performance via quality assessment performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided through the Organization.

    13.Performs other duties and activities as delegated by the Clinical Director/Director of Patient Care Services.

    Position Qualifications

    1. Registered nurse with current licensure to practice professional nursing in the state, with at least 2 years of hospice, home health, or community health experience in the last years.

    2. Graduate of National League of Nursing accredited school of nursing with a BSN degree is preferred.

    3. Excellent understanding of performance improvement, quality assessment, and
    utilization management.

    4. Complies with accepted professional standards and practice.

    5. Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.

    6. Understands principles of pain/symptom management.

    7. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

    8. Complies and maintains current CPR certification.

    9. Demonstrates excellent observation, verbal and written communication skills.

    10. Preferably able to speak Chinese but not required.

    Job Type: Full-time

    Pay: $80,000.00 - $120,000.00 per year
