Hospice Supervisor manages the resources, operations, and staff of hospice care programs. Works with families and patients to develop optimal care plans. Being a Hospice Supervisor coordinates the delivery of a patient/family assessment process and care plan development. Oversees interdisciplinary teams of medical doctors, nursing staff, and other care providers inside and outside the facility. Additionally, Hospice Supervisor maintains an up-to-date knowledge of laws and regulations of federal, state, and regulatory bodies relative to hospice care. May require a bachelor's degree in nursing, social work or equivalent. Typically reports to a manager or head of a unit/department. The Hospice Supervisor manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Hospice Supervisor typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required.     Essential Functions

    • Delivery of Patient Care
      • Participates in the assessment of the patient’s physical and mental health, including the patient’s reaction to illness and treatment regimens.
      • Records and reports the results of the nursing assessment.
      • Participates in implementing the Plan of Care developed by the registered nurse and/or prescribed by any person authorized by State law, by performing tasks assigned or delegated by and performed under the supervision or under orders or directions of a registered nurse, licensed physician or any other person authorized by State law to provide supervision.
      • Facilitates patient and caregiver independence to the extent possible.
      • Participates in the teaching and counseling of patients as assigned by a registered nurse, physician or other qualified professional authorized by State law.
      • Encourages and allows time for questions when meeting with the patient and family.
      • Reports and records the nursing care rendered, and the patient’s response to that care.
      • Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
      • Maintains open lines of communication with supervisor. Keeps supervisor apprised of any complex patient/caregiver issues.
      • Applies principles of infection control. Follows guidelines for universal precautions in accordance with accepted professional practices.
    • Interdisciplinary Group 
      • Attends and participates in the interdisciplinary group conferences [IDG] and special care conferences.
      • Seeking and utilizing the appropriate resources for the referral process, as directed by the RN
      • Participates in identifying specific patient/caregiver needs and making referrals to appropriate team members.
    • Documentation
      • Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.
    • Professional and Personal Accountability
      • Demonstrates competency in knowledge and skills within scope of practice.
      • Ensures that VIA Health Partners professional reputation is maintained and projected.
      • Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.
      • Maintains current professional licensure, CPR certification and CEU requirements.
      • Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to unforeseen obstacles.
    • May perform other duties as required.

     

    Qualifications


    Minimum Qualifications

    • A Diploma from a Board approved nursing program designed to prepare a person for practical nurse licensure is required;
    • A current, active, and unrestricted compact (multistate) license as a Licensed Practical Nurse (LPN) is required.  
    • The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.
    • Current CPR required; CHPN preferred.
    • Minimum 1-year related experience for LDHH and 2 years for other nursing positions.  Experience in direct care of patients with life limiting illnesses required.  Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
    • Computer proficiency is required.
