Position Summary 

Full-time (35hrs) case management supervisor staff position, offering flexible scheduling Monday through Friday 9am to 7pm, able to work days and evenings.  Services are delivered in person and via Telehealth, providing case management to clients, ages 18 to 100 , to assist with navigation of barriers with goals of improving social determinants of health e.g. employment, economic, education, health, housing and social supports. Case management services are to empower clients with leading, enriched lives as they cope with issues throughout the life cycle including but not limited to mental health, medical health, substance use recovery and the aging process.   Services provided are trauma informed, harm reduction orientated, culturally competent and LGBTQAI affirmative.
 

Primary Responsibilities

• Conduct case management services via in person in office, offsite and/or Telehealth
• Provide direct case management services to include:   application/renewal of benefits and entitlements; referral to appropriate community resources, e.g. securing identification, employment, legal, housing, healthcare services; and supportive escort to medical, legal, social service and health related appointments.
• Provide supervision to an interdisciplinary team of 8 staff composed of: home health attendants, service care coordinators and case managers overseeing the delivery of direct services.
• Participate in weekly supervision, and interdisciplinary staff meetings.
• Timely completion of documentation in electronic health record and other required reports to comply with:  NYS-OASAS, OMH & OCF, as well as, NYC-DOH MH regulations and requirements to include but not limited to: incident reports, progress notes: and encounter forms.  
• Report to the administration about recipient’s unmet needs.
• Serve on the agency’s committees as assigned.
• Immediate notification of Directors and appropriate clinical staff of reportable incidents and at risk behaviors. 
• Participate in ongoing professional training for continuing education units and other professional development as mandated by the agency or recommended by supervisor.
• Perform other tasks and special projects as assigned by the direct supervisor.

Secondary Responsibilities

• Collaborate with other Peer Advocates, Clinicians, Psychiatric Providers and Home Health Aides on shared clients
• Maintain working relationships as needed with entitlement sources, job placement agencies, legal and housing advocates and other social service organizations.

Administrative Responsibilities

• Participate in supervision and interdisciplinary staff meetings.
• Meet program productivity benchmarks for services as determined by agency standards
• Participate in ongoing professional training for continuing education units and other professional development as mandated by agency or recommended by supervisor.
• Perform other tasks as assigned by direct supervisor
• Interact with all levels of personnel within the organization.

Qualifications

• NYS OMH Certified Recovery Peer Advocate with NYS OMH Peer Specialist Certification or NYS OMH Certified Recovery Peer Advocate willing to get NYS OMH Peer Specialist Certification within 9 months of hire.
• Experience with substance use disorder, mental health, and trauma-informed care using a harm reduction approach to recovery.
• Fluent with benefits and entitlements
• Must comply with the agency’s standard of ethics, conduct and organization policies.
• Work collaboratively with interdisciplinary team to accomplish program goals and adapt to the changing program environment.
• Must be able to establish priorities and a course of action for handling multiple tasks.
• Bilingual English/Spanish a plus.
