Concern Housing is seeking a HUD VASH Program Supervisor who will be responsible for providing oversight and supervision to HUD VASH case managers, who work to provide housing services and support directly to Veterans in the HUD VASH program. The Program Supervisor will ensure compliance with all documentation requirements as dictated by the Veterans Administration, utilizing their electronic health record system.
Position: HUD VASH Program Supervisor
Location: Amityville, NY
Schedule: Monday-Friday; 9am-5pm
Duties and Responsibilities:
-Provide weekly supervision to the HUD VASH case managers to ensure the provision of supportive services according
to Veterans’ identified acuity levels and needs.
-Ensure progress notes, assessments, plans, etc. are in compliance with the Veterans Administration documentation
requirements in the Computerized Patient Record System (CPRS). Review and sign-off on documentation in CPRS in
accordance with VA requirements.
-Provide assistance with housing services, including assisting Veterans in obtaining a VASH voucher, and working to identify available housing options in the community.
-Participate in the resolution of landlord and lease issues, lease negotiations, and coordination with public housing
authority recertification's.
-Provide crisis intervention and on-call support, as needed.
-Attend weekly supervision and monthly staff meetings.
-Remain current with all Agency required compliance documents and Relias trainings.
-Other tasks as assigned by the Program Director.
-Positions Supervised: Case Managers
Requirements: Licensed Clinical Social Worker, plus 2 years supervisory experience. Experience in case management, working with Veterans, and familiarity with the Veterans Administration a plus.
