Overview:
Through positive relationship building and managing transitions, the Youth Case Manager helps youth identify and achieve goals related to maintaining safe, stable housing, employment, education, and increasing life skills. In coordination with the Host Home Coordinator, the Youth Case Manager will develop professional relationships and partnerships with other community service providers and provide community engagement or program outreach as required.
