    POSITION SUMMARY
    Provide outreach and case management services for individuals in the community who are experiencing homelessness and living with a severe and persistent mental health issue or co-occurring mental health and substance use disorder who are not service connected.
    RESPONSIBILITIES
    1.     Provide outreach in the community.
                 a.   Conduct outreach activities in the community where people who are experiencing homelessness and living with mental illness congregate.
                 b.   Conduct in reach activities at the local homeless shelters, the ELCA, and Grace Welcome Center.
                 c.   Go into the community and search for people who are living outdoors to assess for need and help people connect to appropriate resources.
                 d.   Meet with adults with MH/SA issues at places they are familiar with to help them access needed resources and services.
                 e.   Conduct appropriate assessment to engage with Kenosha’s Coordinated Entry System for information and referral to
                 f.   Inform and assist individuals connect to the services provided by the KHDS Mental Health and Substance Abuse Resource Center.
                 g.   Provide advocacy to people with MH/SA issues in receiving needed services.
                 h.   Participate in the bi-annual Point-in-Time Event.
                 i.    Provide information to staff of community agencies on mental health and substance abuse (MH/SA) to better serve individuals who are at risk of homelessness.
                 j.    Utilize HMIS as required to capture data and generate reports to account for services being provided.
                 k.   Keep current with all required trainings.
    2.     Ensure documentation is complete and accurate.
                 a.   Use Client Tracking System to document participant contacts and progress.
                 b.   Complete data entry to the Homeless Management Information System (HMIS) as required.
                 c.   Complete and maintain participant files and ensure the information matches the data entered in HMIS.
                 d.   Complete and file discharge plans appropriately.
                 e.   Use the Homeless Assistance Services (HAS) Document to record outreach notes for unknown clients and other non-client activities.

    3.     Provide individualized case management services to eligible program participants on a regular and consistent basis as determined by the individual’s case plan.
                 a.   Enroll eligible people into the PATH program in accordance with program requirements.
                 b.   Assess, plan, coordinate, implement and evaluate the overall service delivered to the participant.
                 c.   Help the participant locate housing and learn to live in housing, maintain their housing in a safe manner, and get along with fellow tenants and the landlord.
                 d.   Create a comprehensive service plan with the participant and provide appropriate interventions to assist with goal achievement.
                 e.   Help participants create support systems and participate in the community, as they desire.
                 f.   Provide individualized budgeting and money management services to program participants as needed.
                 g.   Teach basic, intermediate and advanced social skills based on need.
                 h.   Collaborate with Landlords and other Community Service Providers.
                 i.    Coordinate opportunities for educational advancement, such as GED preparation and attainment, post-secondary training, and vocational education as needed.
                 j.    Assist with job preparation and attainment, such as career counseling, job preparation training, dress and grooming, job placement and job maintenance.
                 k.   Provide assistance in accessing mainstream benefits, including but not limited to food stamps, SSI/SSDI and health insurance.
                 l.    Assist participant in accessing Mental Health, AODA and medical services as needed.
                 m.  Ensure participants have access to food and clothing.
                 n.   Provide transportation using your own personal vehicle or an available agency vehicle and assist in developing transportation skills.
                 o.   Work with the participant to establish a discharge plan prior to program exit and work with that participant intensively to ensure exit to self-sufficiency and housing stability.
                 p.   Required to facilitate 10 groups each grant cycle.


    QUALIFICATIONS:  Bachelor’s Degree in a human service-related field and/or experience working with individuals and families living with mental illness and experiencing homelessness.
    1.     Ability to effectively communicate, both written and verbally, in the English language
    2.     Must have reliable transportation and valid driver’s license with good driving record
    3.     Must carry own auto insurance with at least 100,000/300,000/100,000 coverage
    4.     Ability to maintain records and document case notes daily
    5.     Ability to give and accept feedback as a part of a team
    6.     Ability to traverse within the workspace and community frequently
    7.     Be willing to learn and work with the treatment philosophies
    8.     Bilingual in Spanish a plus
