IT Capacity Planner, Sr. evaluates the organization's data processing and storage needs and develops long-term plans to meet business requirements. Implements policies for network storage system troubleshooting, updating and general maintenance. Being an IT Capacity Planner, Sr. documents problems and solutions and serves as the point of contact for network capacity related issues. Allocates storage space, plans for future projects or acquisitions, reviews hardware and software configurations and makes suggestions for appropriate storage or backup models. Additionally, IT Capacity Planner, Sr. may require a bachelor's degree in area of specialty. Typically reports to a manager. To be an IT Capacity Planner, Sr. typically requires 4 to 7 years of related experience. Contributes to moderately complex aspects of a project. Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotics). Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options. Conduct and coordinate clinical investigations and research (consistent with other healthcare professionals) approved by a local or regional Investigational Review Board (IRB) and participating in outcome studies generated by the Pharmacy Flight and pproved by the P&T Committee. Document significant medication interactions, and pharmacy interactions with prescribers. Conduct Adverse Drug Reaction (ADR) reporting in accordance with TJC standards as appropriate. Facilitate medical staff drug utilization reviews (DURs) as deemed necessary.
