The Care Coordination Engagement Supervisor is primarily responsible for ensuring adherence to the high-fidelity wraparound model of care coordination, ensuring quality care for children and families/caregivers representing diverse socioeconomic, racial, ethnic, and cultural backgrounds. This position will be primarily responsible for leading a team that will support families in the OhioRISE program who are being discharged from a hospital due to a behavioral health condition. The supervisor has a thorough understanding of local community resources and is skilled at developing and maintaining working relationships with staff, clients, and community partners. This position will be responsible for serving Region I (Fairfield, Perry, Muskingum, Morgan, Noble, Guernsey, Coshocton, and Washington Counties).
LSW/LPC required, LISW/LPCC strongly preferred.
The salary range for this position is $52,609.67 to $79,175.95.
