    The incumbent serves as a CC&ICM Subject Matter Expert to Social Work Departments. They provide analysis and evaluation of program data and computerized programs to identify system-wide trends and needs to enhance the quality of service across all programs within CC&ICM framework.

    Provides Social Work Practice Expertise and Technical Assistance:

    The incumbents are the designated experts concerning the provision of case management services. The incumbents will assist less experienced and less knowledgeable staff with technical expertise and guidance for patients with unusual or complicated medical and mental health conditions, risk factors, and ethical or legal circumstances. The incumbents will provide this assistance for Social Work, and other staff as needed or requested. This includes the interpretation of VHA Directives, Medical Center Policies, Social Work Practice/Case Management, Program Handbooks, and applicable state laws and guidelines.

    The cases presented to the incumbents are often very complicated psychiatric, social, and medical issues including patients in acute, clinical, residential, and community settings involving conditions with complicating psychosocial factors. The incumbents will regularly address the most difficult, challenging, complex, and acute patient care needs encountered in the Medical Center treatment areas and communities served. They will coordinate and collaborate with the facility level case management teams to include nurse and social worker case managers. The incumbents establish collaborative relationships with physicians, psychologists, pharmacists, and other disciplines to foster their involvement in the case management of Veterans. They advocate on behalf of case management services and facilitate communication with facility executive leadership on case management strengths and challenges.

    Collaborates with internal stakeholders to assess case management composition, structure, and services to identify promising practices and gaps in case management services. Identify opportunities to enhance communication, collaboration, and coordination across the care continuum.

    Education, Coaching, Consulting, and Mentoring:

    Directs/coordinates clinical and psychosocial services and is accountable for the overall effectiveness of the services provided. Modifies services to best meet the treatment needs of veterans and to promote efficient practice. Social Work services with other services offered in the treatment program, assuring such services are complementary and comprehensive. Directs activities to maximize effectiveness, efficiency, and continuity of care for veterans. Provides case management services to those veterans at high social, psychiatric, or medical risk, serving as the liaison to other treatment programs. Represents the program in contacts with other agencies and the public.

    Provides orientation and on-going training to nurses, social workers, and interdisciplinary team members. Conducts staff development programs for nurses and social workers and takes responsibility for providing opportunities to help staff update their Social Work practice skills and acquire new knowledge in contemporary treatment modalities. Facilitates and/or coordinates training on nationally recognized case management standards of practice to ensure delivery of professional case management services across the healthcare continuum.

    Promotes the benefits of case management certification for facility case managers. Develops and facilitates educational programs informing the Veteran consumer and family members regarding available case management services.

    The incumbents develop, plan, and facilitate in-service training programs and works with individual staff to identify learning needs and necessary educational programs and literature to meet their needs in order to develop their professional/clinical knowledge and skills. Incumbents provide education and training to staff from other disciplines, students, and volunteers as assigned. Develops, provides, and participates in interdisciplinary staff education programs and team-building activities.

    Qualifications

    Basic Requirements:
    Citizenship. Be a citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    Education. Have a Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Please note: A doctoral degree in social work may not be substituted for the master's degree in social work.
    Licensure. Licensed or certified by a state to independently practice social work at the master's degree level. VHA may waive the licensure requirement for persons who are otherwise qualified, pending completion of state prerequisites.
    English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Regulations.

    May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Must pass a background check.

    Grade Requirements:
    GS-13 Social Worker (Program Coordinator)
    Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
    Licensure/Certification. Individuals assigned as supervisory social workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
    Demonstrated Knowledge, Skills, and Abilities
    1. Skill in coordinating and implementing one or more specialty program(s), across the facility.
    2. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
    3. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
    4. Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
    5. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
    6. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
    7. Knowledge of organizational structure and impact on program operations.

    Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Supervisory social workers at the GS-13 grade level typically supervise professional and non-professional staff. They are responsible for the professional and administrative management of an area in a social work service, in a care/product line, or across multiple sites, (e.g., multi-division facilities and CBOCs).
